This is the NSPS Subpart QQQ report covering the period of July 1,2024, to December3l,2024
for Tesoro Refining & Marketing Company LLC's Salt Lake City Refinery and Tesoro Logistics
Operations, LLC.
This report includes information required by ApprovalOrder DAQE-AN103350075-18, and Title V
Operating Permit 3500008002, conditions II.B.5.a-f.
This report summarizes inspections for the semi-annual period when a water seal was dry or
otherwise breached, or when a drain cap or plug was missing or improperly installed, or when
cracks, gaps, or other problems were identified that could result in VOC emissions, and includes the
corrective actions taken.

Other Inspection ltems
o A routine inspection of the oily drain S-2 (laboratory sink) identified a missing water seal
due to a leak in the under-sink p-trap. The water faucet was opened to immediately
reestablish a water seal, a leak pan was placed under the p-trap, and the sink was removed
from hydrocarbon service until a maintenance work order was executed and the leaking p-
trap was repaired.
r During an inspection of the Flare Gas Recovery Unit area drains, the water seal for drain
hub DH-5 could not be visually verified. A Method 2l inspection was performed, and VOC
emissions did not exceed 500 ppm. In an abundance of caution, a drop-in vertical pipe trap
was subsequently installed to ensure ease of visual inspection.
