(Parkland), as a summary of the high vacuum dual phase extraction (HVDPE) event and groundwater sampling data collected from well MWH-2 at the above-referenced facility. The scope of work was conducted according to the Work Plan submitted to the Utah Department of Environmental Quality Division of Environmental Response and Remediation (DERR) on March 14, 2024, and approved by the DERR on April 10, 2024. Summary of Background Information The Hart’s Gas and Food site is located at 335 W 7200 S, Midvale, Utah. The elevation of the site is approximately 4,383 feet above mean sea level and the topography of the area slopes to the west. The site is currently an active retail gas station with a station building, fueling canopy, and six dispenser islands. The site is paved with asphalt and concrete. A site map is included as Figure 1. The site has been reported to have been occupied by a retail gasoline service station which originally utilized three steel Underground Storage Tanks (USTs) since 1971. The steel USTs were closed and removed from the site in October 1998. During closure activities, contaminated soil was discovered in the UST basin area. A release was reported to the DERR on October 26, 1998 and was assigned release ID # KRP. Impacted soil was over-excavated and hauled off-site. Release KRP was issued a No Further Action (NFA) letter in September 1999. In December 1998, four fiberglass reinforced plastic USTs were installed within the original tank basin. No known releases have occurred from the current UST system. In January 2000, a release was discovered at the Chevron gasoline station located to the west across Cottonwood Street. Numerous subsurface investigations and groundwater monitoring events have occurred at the Chevron facility to define and evaluate the magnitude and extent of the release. To help delineate the Chevron facility’s release, three sets of nested monitoring wells (MW-14/MW-14A, MW-15/MW-15A, MW-16/MW-16A) were installed on Midvale City Right-of- Way (sidewalk), located within close proximity to the Hart’s western property boundary. Nested monitoring wells MW- 14/MW-14A, MW-15/MW-15A, MW-16/MW-16A have historically provided intermittent groundwater for the collection of water samples and laboratory analysis. When groundwater has been present, analytical results have indicated contaminants of concern (CoCs) in samples collected from MW-14 exceeding DERR Initial Screening Levels (ISLs) and water collected from MW-14A containing CoCs exceeding Tier 1 screening levels. On October 2016, the DERR directed a State contracted consultant to perform a subsurface investigation on the Hart’s property to evaluate on-site conditions for a potential source of the identified CoCs in MW-14 and MW-14A. The subsurface investigation included five soil borings using direct push technology. All borings were completed with UDEQ DERR Project Manager Utah Department of Environmental Quality Division of Environmental Response and Remediation NFB-06 HVDPE and Groundwater Monitoring Report Page 2 www.anteagroup.com monitoring wells (MWH-1 through MWH-5). Due to insufficient groundwater volumes in MWH-1 through MWH-4, groundwater samples were only collected from MWH-5. While the report concluded that no specific source was identified, on-site soil impacts were detected above Tier 1 Screening Levels in the MWH-3 boring at a depth of 22 feet. Results from the subsurface Investigation can be found in the Subsurface Investigation Report submitted on January 20, 2017 by ATC Group Services. On June 18, 2018, a groundwater monitoring event was performed by Underwood Environmental. The monitoring event included gauging two wells on the adjoining Chevron site (MW-1, MW-7) one well in the 7200 South right-of-way (MW- 6), eight wells in the Cottonwood Street right-of-way (MW-8, MW-14, MW-14A, MW-15, MW-15A, MW-16, MW-16A, MW-19) and four wells on the Harts site (MWH-1, MWH-2, MWH-3, and MWH-4). Additionally, well MWH-5 located across 7200 South to the northwest was gauged. Of the 16 wells gauged, only wells MW-6, MW-7, MW-14, MWH-2, and MWH-5) contained sufficient water to collect samples. Laboratory results indicated that monitoring wells MW-14 and MWH-2 exceeded the (ISLs) for one or more hydrocarbon constituents. MW-14 detected benzene at 0.008 mg/L and MWH-2 detected benzene at 0.018 mg/L, Total Petroleum Hydrocarbons as Gasoline (TPH-GRO) at 1.48 mg/L, and Total Petroleum Hydrocarbons as Diesel (TPH-DRO) at 2.5 mg/L. The groundwater flow direction was determined to be towards the south-southeast. On October 12 and 13, 2020, Antea Group conducted an on-site soil investigation to further evaluate the extent of the on-site impacted soil. The investigation consisted of the advancement of five soil borings using direct push technology. Soil samples were collected continuously beginning at five feet and were screened for volatile organic compounds using a Photoionization Detector (PID). Three soil samples were collected for laboratory analysis from each soil boring within the zones of highest PID readings or at the bottom of the boring. Soil analytical results detected COCs above ISLs and Tier 1 Screening Criteria in soil samples collected from soil borings SB-1, SB-2, SB-3, and SB-4. Soil analytical data is included as Table 1. Additionally, monitoring wells (MWH-1 through MWH-5) were gauged and sampled on October 14, 2020. MWH-3 and MWH-4 were gauged as dry and were not sampled. Groundwater samples were submitted to Pace National of Mount Juliet, Tennessee to be analyzed for Benzene, Toluene, Ethylbenzene, Xylenes, Naphthalene (BTEXN), Methyl Tert-Butyl Ether (MTBE), TPH-GRO, and TPH-DRO using EPA Methods 8260 and 8015. Groundwater analytical results indicated that MWH-2 exceeded Tier 1 Screening Criteria for TPH-DRO at 16.5 mg/L and exceeded ISLs for Benzene at 0.100 mg/L and TPH-GRO at 3.04 mg/L. On June 16, 2021, groundwater monitoring of wells MWH-1 through MWH-5 gauged and sampled. Monitoring wells MWH-3 and MWH-4 were gauged as dry and could not be sampled. Wells MWH-1, MWH-2, and MWH-5 had detectable groundwater at depths between 30.91 in MWH-5 and 37.03 in MWH-1. The observed intermittent occurrence of groundwater in the on-site wells indicated the saturated zones observed in MWH-1 and MWH-2 were not continuous, and a groundwater flow direction across the well network could not be determined. In MWH-2, 0.17 feet of Light Non- Aqueous Phase Liquid (LNAPL) was observed. Groundwater samples from MWH-1, MWH-2, and MWH-5 were submitted to Pace National of Mount Juliet, Tennessee, and analyzed from BTEXN, MTBE, TPH GRO, and TPH DRO using EPA Methods 8260 and 8015. In MWH-1 and MWH-5 no COCs were detected above ISLs. Benzene exceeded Tier 1 Screening Criteria in MWH-2 at a concentration of 0.455 mg/L. TPH-GRO exceeded ISLs at MWH-2 at a concentration of 3.36 mg/L. The groundwater sample from MWH-2 received a laboratory “J2” TPH-DRO qualifier, which indicated that the surrogate recovery limits had been exceeded, and values were outside lower control limits. On December 14, 2021, Monitoring wells (MWH-1 through MWH-5) were gauged and sampled. Wells MWH-3 and MWH- 4 were gauged as dry and were not sampled. Groundwater samples collected from MWH-1, MWH-2, and MWH-5 were submitted to Pace National of Mount Juliet, Tennessee, to be analyzed for BTEXN, MTBE, TPH-GRO, and TPH-DRO using EPA Methods 8260 and 8015. Groundwater gauging results detected groundwater in wells MWH-1, MWH-2 and off-site well MWH-5 at depths ranging from 30.53 feet in MWH-5 to 34.02 feet in MWH-1. No groundwater was detected in on- site wells MWH-3 and MWH-4. No LNAPL was detected in any of the wells gauged. Groundwater analytical results did not detect COCs above ISLs in MWH-1 and MWH-5. Monitoring well MWH-2 contained COCs above ISLs for Benzene (0.092 mg/L), TPH-GRO (2.03 mg/L), and TPH-DRO (7.45 mg/L). No COCs in MWH-2 exceeded Tier 1 Screening Criteria. (CalClean) to perform a five-day HVDPE event extracting water and vapor from well MWH-2 on June 24-29, 2024. Included as Attachment 1 is a HVDPE report prepared by CalClean which includes a description of the equipment used to facilitate the work, operational parameters employed to extract water and vapor, vapor monitoring data and calculated hydrocarbon mass removal rates and totals. Antea Group performed groundwater monitoring of MWH-2 on June 24 prior to the HVDPE event and post HVDPE event on July 22 and October 25. Groundwater monitoring data is included in Table 1 and laboratory reports are included as Attachment 2. The applied vacuum during the extraction work ranged from a high of 23 inches of mercury (in. of Hg) on startup and then stabilized at 21 in. of Hg on the second day of extraction and remained steady at 21 in. of Hg for the duration of the 5-day event. Average air flow from the extraction well was 41 standard cubic feet per minute (scfm). Hydrocarbon vapor removal rates increased after system startup and peaked during the second day of extraction and then gradually declined to the end of day 5. Cumulative removal over the 5 day event was 880 pounds of hydrocarbons based on laboratory data of daily samples. A relatively small volume of groundwater was recovered during the extraction event with a total of 330 gallons recovered over the 5-day event. The volume of water recovered was not significant enough to utilize CalCleans water treatment system, so all the recovered water was placed in (6) 55-gallon drums and stored on-site pending transport and disposal at E.T. Technologies. Waste disposal documents are included in Attachment 3. Groundwater gauging and sampling was performed at well MWH-2 before extraction and after the completion of the 5- day extraction event. On June 24 groundwater was gauged at a depth of 33.89 feet and the total well depth was measured at 37.00 feet. A groundwater sample was collected from MWH-2 and submitted to Pace National of Mount Juliet, Tennessee, to be analyzed for BTEXN, MTBE, TPH-GRO, and TPH-DRO using EPA Methods 8260 and 8015. Groundwater analytical data is included as Table 1 and laboratory reports are included in Attachment 2. Analytical results for MWH-2 indicated COCs above ISLs including Benzene (0.0812 mg/L), and TPH-DRO (7.67 mg/L). No COCs in MWH-2 exceeded Tier 1 Screening Criteria. Post extraction gauging performed in July and October has not detected groundwater in MWH-2, indicating the thin saturated zone has been dewatered by the extraction event and is very slow to recover. Additional groundwater gauging of MWH-2 will be performed in 2025 to evaluate if groundwater will recover in MWH-2 post extraction event. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description J The identification of the analyte is acceptable; the reported value is an estimate. J4 The associated batch QC was outside the established quality control range for accuracy.
