Mathis Senior Support Officer - Joshua Greer April 4, 2025 State Fuel Network Attn: Brett Gurney 4215 S 2700 W Taylorsville UT 84129 Re: Closure Plan approval for Underground Storage Tanks (USTs) Mountain Crest HS Consolidated – ID 100505 Dear Mr. Gurney, The closure Plan for the above-referenced facility, received by the Bear River Health Department on April 4, 2025, has been approved subject to the noted modifications, if any. This department and the local fire department must be notified 72 hours before beginning closure activities. The local agencies may have additional requirements for closure or may charge inspection fees (see enclosures). The closure Plan approval is effective for a period of one year from the date of approval. lf closure does not take place within one year, submittal of a new closure plan will be required unless otherwise approved by the Executive Secretary. Any change to the approved closure plan must be submitted in writing and approved by our office before implementation. lf contamination is suspected or found during closure activities you must report it to the Division of Environmental Response and Remediation (DERR) at (801)536-4100 within 24 hours of discovery. Enclosed is a copy of the Closure Notice form which must be completed and submitted to the Executive Secretary (UST). Please submit the environmental and Unified Soil classification (USC) sample analysis data and chain of custody forms with the Closure Notice as soon as possible, but no later than 90 days after tank closure.
