Rule: R309-515-6(12)(d)(ix)The discharge end of the pump-to-waste line shall be covered with a No. 4 mesh
corrosion-resistant screen. This significant deficiency must be corrected within 120
days of notification or have a corrective action plan approved by DDW.
Notes:Screen was blown off at time of survey. Was remedied immediately after.

Rule requires that all chemicals added to water being treated for use in a public water
system for human consumption comply with ANSI/NSF Standard 60. This significant
deficiency must be corrected within 120 days of notification or have a compliance
action plan approved by DDW.
Notes:Citric Acid is food grade, Matt ordered new certified chemcials same day. Points will be
resolved once new chemical is delivered.
