The Utah Division of Environmental Response and Remediation (DERR) is requesting that
Kleinfelder, Incorporated ("Kleinfelder"), under contract#99-6340, complete the enclosed Work Assignment
toconductariskassessmentfortheabove-referencedfacility. Thescopeofwork,timescheduleandbudget
were verbally discussed and mutually agreed upon during a December 23,1998 phone conversation between
the Kleinfelder and DERR project managers. Please sign both copies of the Work Assignment and retum

Tier 2 Risk Assessment and Subsequent Reporting
The contractor, Kleinfelder, Incorporated ("Kleinfelder") is requested to complete the following tasks for
this Work Assignment.
I WORK OBJECTIVES
TheWorkAssignmentobjectiveis toconductaRisk-BasedCorrectiveAction (RBCA)Tier2Risk
Assessment which entails conducting file review as needed, and completion and submittal of a risk
assessment report done in the standard RBCA reporting format as discussed herein, and includes
analyses of known site and subsurface conditions, risk factors, exposure pathways and transport
mechanisms, and other relevant items. The submitted report must also evaluate the need for, and
the associated costs of, site-specific cleanup activities, and provide appropriate site-specific
recommendations for conducting the minimum amount of additional work needed at the site which
may be necessary in order to obtain final site closure.
II TASKS AND BUDGET
The expense to the Department of Environmental Quality (DEQ) for the implementation and
completion of this Work Assignment shall not exceed $7,Sffi.ffi which includes project management,
contract management, and all other costs. Kleinfelder will notify the DEQ when coss for any task
have exceededT5To of thetotal costs specifiedin theapprovedWorkPlanorWorkAssignment, or
if Kleinfelder has reason to believe that the costs will be greater than the approved budget. DEQ
has identified the following specific tasks:
A. Conduct"ProjectManagement" necessary tocomplete the objectives and tasks of this Work
Assignment. The budget for this task is approved at $750.00.
B. A SiteConceptual ExposureModel (SCEM) shallbefilledoutcompletely and submittedto
the DERR project manager within two weeks of your receipt of this Work Assignment.
This initial approval of the SCEM by your project manager will help to ensure that the
resulting risk assessment report will evaluate all the necessary exposure pathways and other
risk characteristics, and minimize the need for any potential reworking of the risk
assessment.
C. TheRBCA evaluation shouldconsidertheduration andresults of thegroundwatermonitoring
program previously conducted at the release site in terms of documenting both gtoundwater
contaminant plume stability and the full extent and degree of the subsurface soil and
groundwater contamination. If plume stability cannot be determined based on existing data,
or minimum data requirements have not yet been met in accordance with the RBCA Tier 2
protocol, thenKleinfeldershall identify the minimumdatarequirements (such as additional
groundwater monitoring events, slug tests, pilot testing, additional soil samples, or other
environmental tests, etc.) required in order to document such release site conditions and
properly complete the risk assessment report.
The corrective action feasibility analysis should consider the results of any abatement
activities (such as enhanced fluid recovery) completed at the site to date, if any have been
completed, in evaluating the cost effectiveness of either initiating or conducting additional
abatement activities in an effort to reduce source concentrations in order to provide adequate
protection to human health and the environment at the release site. The feasibility analyses
should also discuss the advantages and disadvantages of conducting any abatement or
cleanup activities andtheir associatedcosts which may be identified as warranted for the site
during the RBCA evaluation process.
Collect one soil and one groundwater sample at depths and locations which would be
representative of "worst case" contaminant levels. Both samples are to be analyzed for
BTEXN and TPH. For the TPH analysis, follow the RBCA protocol for "fractionation".
Kleinfelder will follow the standard report format as outlined in "Guidelines for Utah's
Corrective Action Process for Leaking Underground Storage Tank Sites", and conduct the
risk assessment using the most appropriate site-specific option within the Tier 2 process."
The submitted report shall inctude all the necessary worksheets and supporting
documentation as described in appendices "A" through "C" of the RBCA document.
REPORTING REQUIREMENTS
A. Kleinfelder is required to report to the DEQ prior to proceeding with any task that will
overrun or can be projected to overrun the cost estimate defined in Section II above.
B. Kleinfelder is required to submit a final RBCA risk assessment report in accordance with
the work assignment objectives by March 31r1999.
CONFLICT OF INTEREST
Kleinfelderwill identify and describe anypossibleconflict of interestin fulfillingtherequirements of
the Work Assignment.

The contractor (Kleinfelder. Incorporated) is requested to complete the following items as Work
Assignment Amendment #EHLR-S.1. The following changes are hereby made to Work Assignment
#EHLR-5, which was originally approved by the DERR on January 5,1999.
Due to the elapsed time since the last groundwater monitoring round at the site in
conjunction with the intent of the Work Assignment, it has come to the DERR's attention
that a current round of samples will need to be taken in order to facilitate the completion
of the risk assessment analyses as required by the original work assignment. The original
work assignment inadvertently did not specify this work or associated budget.
Kleinfelder shall conduct groundwater monitoring from B-5, B-7, B-9 and B-10 using
proper sampling protocol pursuant to Utah Admin. Code R311-205. Groundwater
samples shall be analyzed for the presence of Dissolved Oxygen (DO), Oxygen
Reduction Potential (ORP), Total Petroleum Hydrocarbons (TPH), Benzene, Toluene,
Ethylbenzene, Xylenes, MTBE, and Naphthalene. Perform the TPH fractionation on the
groundwater sample obtained from B-5 as described in the original work assignment.
The reporting for this work assignment amendment shall be combined with the risk
assessment report and should contain a section which details the groundwater conditions
at the site which should include, but not be limited to the following:
the adjusted depth to water;
site map showing well locations, utilities,
relevant items;
structures, property lines and other
groundwater flow direction;
actual laboratory results and chain of custody forms;
isoconcentration maps for benzene and naphthalene in groundwater;
and, any other applicable item associated with typical monitoring reports such
free product thickness.

The sum of $E50.00 is hereby added to the total Work Assignment cost. The total adjusted work
assignment cost approved to date is therefor effectively amended from $7,500.00 to the final approved

Site Status Update and Request for Consent to Access Property
Dear Mr. Christensen:
As you are already aware, the Division of Environmental Response and Remediation (DERR) has
initiated the work necessary to protect human health and the environment at the above-referenced release
site ("Site") and is pursuing the required investigation and remediation with LUST Trust or State funds. The
DERR may then proceed to recover, from responsible parties, all expenditures associated with investigation,
abatement, and corrective action at the Site as provided in Utah Code Ann.. 19-6-420 (2XbXiv).
The investigative phase of the work has now been completed, and the next phase of work currently
in progress is evaluating the site for closure. This risk-based closure evaluation requires the collection of
some additional soil and groundwater samples from the release site to determine current levels of
contamination which remain at the property. The DERR is sending you another, more detailed access
agreement for your signature which extends the time frame to accomplish the described work.
Please sign the access agreement, and return it to the DERR within two weeks of your receipt of this
letter. If you have any questions regarding this matter, please contact the state-assigned Project Manager,

The Owner hereby grants to the officers, employees, authorized representatives and consultants
of the Utah Division of Environmental Response and Remediation ("DERR") access, including ingress
and egress, to the Property for the following purposes:
1. Conduct groundwater and soil monitoring and sampling from the on-site groundwater
monitoring wells. To conduct subsurface investigation studies to determine and define
the extent and degree of subsurface petroleum contamination originating from an
Underground Storage Tank ("UST") system. This may include soil borings, drill or hand
auger holes for soil sampling, and installation of groundwater monitoring wells for water
sampling.
2. Any other such actions related to the definition, or cleanup of the subsurface petroleum
contamination on the Property.
The tasks described above may be altered if conditions change or if the DERR obtains additional
information requiring further investigation. The DERR will notify the Owner in writing of any new
planned tasks.
It is anticipated that the work described herein will be performed between January I,1999 and
December 31, 2000. If it becomes necessary to perform the work at other times, the DERR will notify
the Owner by telephone at least twenty four hours before it intends to enter the Property. By granting
access to the DERR, the Owner makes no admission of liability or responsibility for any petroleum
contamination which may be found on the Property.
This grant of access shall remain in effect until, and only terminate upon, the completion of the
investigation, any clean up activities and any additional tasks resulting from new information as noted
above.

The contractor (Kleinfelder, Incorporated) is requested to complete the following items as Work
Assignment Amendment # EHLR-5.2. The following changes are hereby made to Work Assignment
#EHLR-5, which was originally approved by the DERR on January 5,1999.
Due to upcoming administrative proceedings which may involve apportionment of
liability between potentially responsible parties, the DERR has identified a need to
conduct additional environmental sampling. Specifically, soil samples need to be
collected to test for the presence of total lead. The two soil samples for lead testing
should come from a contaminated area at the on-site source area (near B-5 at the same
location used for TPH fractionation sampling) and at an up-or downgradient "clean" area
in order to determine the difference between levels that may be naturally occurring and
considered "background", and those areas which may have been impacted by the release
of leaded gasoline in the past. The soil and groundwater samples for TPH fractionation
shall be submitted to American West Analytical Laboratories in Salt Lake City, Utah and
marked as "Utah TPH Fractionation" for the analytical method to be used.
Due to the destruction of some of the critical groundwater monitoring wells during recent
construction activities, the final sampling and analysis plan has been modified as shown
below:

3. The reporting for this work assignment amendment shall be combined with the risk
assessment report and should contain a discussion of the lead and fractionation results in
terms of product Upe(s) and other contaminant characteristics which may aid in the
determination of multiple releases from the same property or mixed releases from
different properties to assist the DERR in future cleanup funding and apportionment of
liability decisions.
The sum of $750.00 is hereby added to the total work assignment cost. The total adjusted work
assignment cost approved to date is therefor effectively amended from $8,350.00 (e.g., the amount
authorized by Work Assignment #EHLR-5.l) to the final approved amount of $9,100.00. The work shall
be performed in the same character as provided for in the Contract. This change in the scope of work
changes the original March 31, 1999 report due date to April 19,1999.
