SITE NAME: Chromalox

SITE LOCATION: Ogden

DATE OF SITE VISIT: October 15, 2019

PURPOSE OF SITE VISIT: Observe mixing of hydrogen peroxide and sodium persulfate, trouble shooting
leaks in the lines, daylighting, injections.

FIELD OBSERVATIONS: I arrived onsite and injections had begun a few hours prior into wells within the
main gallery area. RPS will provide how much was injected into each well in the report. Minor leaks in
the line due to copper fitting reacting with the injectate, all leaks were contained in buckets and floors
cleaned. After tote was drained repairs were made. Observed mixing of one batch of injectate: one- 275
galon tote of 12% Hydrogen Peroxide was mixed with six- 25 Kg bags of “Klozur” (Sodium Persulfate).
RPS has 9- totes of Hydrogen Peroxide onsite. Pumping into INJ-5 at low pressures resulted in
breakthrough/ surfacing of injectate 25-30 west of the well on two occasions.
