SITE NAME: Chromalox

SITE LOCATION: Ogden

DATE OF SITE VISIT: September 12, 2019

PURPOSE OF SITE VISIT: Observe drilling of monitoring wells, setting wells, and collecting soil samples

FIELD OBSERVATIONS: Fieldwork began with Earthprobe (drillers) on 9/11/19 with the instillation of
MW-12 shallow and deep. Todays work moved inside of the building to the location of MW-14. The
deep well was geoprobed first to log and collect samples. After logging a 6 3/4” auger was advanced to
40’ to set the 2” PVC well (screen 35-40’). When moving the auger flight up it appears a sand bridge
occurred and also pulled up the PVC. The PVC had to be completely removed from the boring and the
boring redrilled in order to set the well. The shallow well was then logged and sampled at MW-14.
