SITE NAME: Chromalox

SITE LOCATION: Ogden

DATE OF SITE VISIT: March 30, 2022

PURPOSE OF SITE VISIT: Observe low-flow groundwater sampling.

FIELD OBSERVATIONS: I arrived onsite and signed into the office. Larry was setting up on GMW-4 north
of the property in the field. Groundwater was purged at the lowest rate possible for the pump. All
parameters and drawdown were stabilized before sampling. Samples were collected in preserved
containers and placed on ice. Metals were filtered in the field. No issues were observed with the
sampling.
