15-Day Notification of Project Cancellation

Ms. Jerauld,

Merit Medical (Merit) Notice of Intent (NOI) does not meet the requirements of R307-401-5. Merit was
notified of the deficiencies on June 21, 2024, with the following discussion on September 25, 2024. The
Utah Division of Air Quality (DAQ) has not received the required information as of the date of this letter.

Merit must provide:
• The economic, energy, and environmental costs for the afterburner.
• The costs of the other lower-emitting combustion technologies that were not proposed
• An overall emissions summary, including each HAP and CO2e emissions.
• A map of the site, indicating where the new afterburner will be located.
• Modeling information that conforms with R307-410.
The original NOI was submitted on May 23, 2024. The DAQ notified Merit of the deficiencies on June
21, 2024. On September 25, 2024, we requested that Merit submit the required information within two
weeks.

Because the necessary information has not been received, Merit must submit the required information
within 15 days from the date of this letter. If the information is not received within 15 days from the date
of this letter, the DAQ will cancel this project.
