Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. This review will update the formatting and language of this AO to make it similar to modern permits. The permit requirements are subject to change if the state or federal laws referenced have changed since the permit was issued. You or your company will not be charged anything for this project. Additionally, if this site is no longer operational the permit can be revoked. Please respond to the following questions as soon as possible. Is the site still operational? Take highway 10 south to Utah 122 and turn west toward Hiawatha (approx. 7 miles south of Price). Go west 2 miles and turn right at the sign for Wattis. Travel approximately 1 mile North. Immediately after crossing the small bridge, take the first paved road to the left. Follow signs to the office. Route 6 to exit 241. Highway 10 to Hiawatha (approx. 7 miles south of Price).
