On August 22, 2024, the Division of Air Quality (DAQ) received your request for a small source
exemption for Shriners Children's. The source is located at 1275 East Fairfax Road, in Salt Lake City, Salt Lake County. DAQ has determined the small source exemption applies to the source, as long as the
equipment and associated processes operate as specified in the registration request.
The small source exemption does not exempt a source from complying with other applicable federal,
state, and local regulations and the current Utah Administrative Code. Based on the emissions that you
submitted to DAQ with your registration request, Shriners Children's Medical Clinic is not required to
obtain an approval order under R307-401. If you change your operation such that there is an increase in
emissions, we recommend that you notify us, as an approval order may be required.
As authorized by the Utah Legislature, the fee for issuing this small source exemption is a one-time filing
fee in addition to the actual time spent by the review engineer and all other staff on the project. Payment
should be sent to DAQ upon receipt of the invoice.

OPERATING STATUS: Boiler #1 was in operation during this inspection.
PROCESS DESCRIPTION: Shriners is a Pediatric Special Care Hospital and currently operates as a
Clinic. The boilers are used to provide comfort heat, and steam to the
facility. Boiler #3 is out of service. Boilers #1 and #2 operate alternately
each month; only one boiler runs at a time for a month, while the other
serves as backup and undergoes testing and maintenance for one hour for
that month. Three water heaters provide hot water to the facility.
The Emergency Generator is used for emergency purposes only and in
the event of a power outage.

Status: In compliance. The above source location information is correct
2. Definitions of terms, abbreviations, and references used in this AO conform to those used in the Utah
Administrative Code Rule 307 (UAC R307), and Series 40 of the Code of Federal Regulations (40
CFR). These definitions take precedence, unless specifically defined otherwise herein.
Status: This is not a compliance issue.
3. Shriners Hospitals for Children shall install and operate three natural gas and diesel fuel fired boilers,
two rated at 8.4 x 106 Btu/hr, and one at 6.3 x 106 Btu/hr, and shall operate one diesel fired backup
generator rated at 1332 bhp according to the terms and conditions of this AO as requested in the
Notice of Intent dated and additional information submitted to the executive secretary dated April 13,
1998.
Status: In compliance. Boiler #3 is currently out of service. Boilers #1 and #2 operate alternately each
month; only one boiler runs at a time for a month, while the other serves as backup and
undergoes testing and maintenance for one hour for that month.
4. At least once per calendar year, all employees who operate equipment (operator) that produces and/or
controls emissions to the air shall receive proper training as to their responsibilities in operating that
equipment according to all relevant conditions of this AO. The training for each operator shall be for
all equipment that operator operates. The equipment shall include all of the equipment listed below in
Condition #5. Within 60 days of every time this AO is modified or reissued, those employees who
operate equipment that produces and/or controls emissions to the air that is affected by the AO
changes shall receive proper training as to their responsibilities in operating equipment according to
all relevant conditions of this AO. Within 60 days of a new operator being employed or assigned
with the job responsibility to operate any of the equipment that produces and/or controls emissions to
the air, the new operator shall receive proper training as to their responsibilities in operating the
equipment according to all relevant conditions of this AO. Records of operator training shall be made
available to the executive secretary or executive secretary's representative upon request and the
records shall include the two-year period prior to he date of the request. This AO shall be made
available to all employees who operate the equip ent listed in this AO.
Status: In compliance. Shriners Hospital train all employees who operate the boilers and the
emergency generator. During this inspection, Mr. Prewitt was the only one trained to operate
the boilers and the emergency generator.
5. The approved installations shall consist of the following equipment or equivalent*:

Status: In compliance. The dual-fueled Boilers #1 and #2 are rated 8.03 MMBTU, were manufactured
in 1994. Boiler #3 in AO Condition 5B is currently out of service. The diesel-fueled Cat 3508
Emergency Generator is rated at 800 kW and was manufactured in 1995.
6. The executive secretary shall be notified in writing upon start-up of the installation, as an initial
compliance inspection is required. Eighteen months from the date of this AO the executive secretary
shall be notified in writing of the status of construction/installation if construction/installation is not
completed. At that time, the executive secretary shall require documentation of the continuous
construction/installation of the operation and may revoke the AO in accordance with R307-1-3.1.5,
UAC. If construction is complete and operation has commenced, a notice is not required on the status
of the construction/installation.
Status: In compliance. According to the records in the DAQ files, the initial inspection was conducted
in August 2000.
7. Visible emissions from the following emission points shall not exceed the following values:
A. Visible emissions from the boiler stacks shall not exceed a 10% opacity limit when using natural
gas as a fuel.
B. Visible emissions from the boiler stacks shall not exceed a 20% opacity limit when using diesel
fuel.
C. Visible emissions from the emergency generator stack shall not exceed a 20% opacity limit.

Opacity observations of emissions from stationary sources shall be conducted according to 40 CFR
60, Appendix A, Method 9. Visible emissions from mobile sources and intermittent sources shall use
procedures similar to Method 9, but the requirement for observations to be made at 15-second
intervals over a six-minute period shall not apply. Any time-interval with no visible emissions shall
not be included.
Status: In compliance. There were no visible emissions observed from the Boiler # 1 during this
inspection. The Emergency Generator was not in operation during this inspection. See the
attachment for additional information.
8. The following production and/or consumption limits shall not be exceeded without prior approval in
accordance with R307-1-3.1, UAC:
A. 62 x 106 standard cubic feet of natural gas per year (all boilers combined) per rolling 12-month
period
B. Operation (including periods for testing/maintenance) shall not exceed 100 hours per boiler per
rolling 12-month period when operated on the diesel fuel #2
C. Emergency generator operation (including periods for testing/maintenance) shall not exceed 300
hours per rolling 12-month period
Compliance with the annual limitations shall be determined on a rolling 12-month total. The
owner/operator shall calculate a new 12-month total based on the first day of each month using data
from the previous 12 months. Records of consumption/operation (utility bills, purchase receipts,
hours of operation from hour meters or operation logs) shall be kept for all periods when the plant is
in operation. Records of consumption/operation, including rolling 12-month totals shall be made
available to the executive secretary or executive secretaries representative upon request and shall
include a period of two years ending with the date of the request. Consumption shall be determined
by company records or billing statements.
Status: In compliance. The rolling 12-month consumption totals/hours of operation from January to
December 2021 included the following.
A. 7.8 x 106 standard cubic feet of natural gas (all boilers combined);
B. 12 hours of operation for testing/maintenance of Boilers #1 and #2 on #2 diesel fuel;
C. 12.32 hours of operation for testing/maintenance emergency generator. .
The Emergency Generator is tested on Fridays for ten minutes with a one hour load test the
second Friday of each month.

Fuels
9. The owner/operator shall use only natural gas as a primary fuel and #2 diesel fuel as a backup fuel in
the boilers. The owner/operator shall use only #2 diesel fuel in the diesel generators. If any other
fuel is to be used, an AO shall be required in accordance with R307-1-3.1, UAC.
Status: In compliance. Shriners Hospital uses natural gas as primary fuel and #2 diesel fuel as
backup for the boilers; and #2 diesel fuel in the emergency generator.

10. The sulfur content of any fuel oil or diesel burned shall not exceed 0.5 percent by weight. Sulfur
content shall be decided by ASTM Method D-4294-89, or approved equivalent. The sulfur content
shall be tested if directed by the executive secretary.
Status: In compliance. According to Sevier Valley Oil, the diesel is classified as Ultra Low Sulfur with
a sulfur content less than 0.0015 sulfur by weight. See the attachment for additional
information.
Records & Miscellaneous
11. All installations and facilities authorized by this AO shall be adequately and properly maintained. All
pollution control vendor-recommended equipment shall be installed, maintained, and operated.
Instructions from the vendor or established maintenance practices that maximize pollution control
shall be used. All maintenance performed on equipment authorized by this AO shall be recorded, and
the records shall be maintained for a period of two years. All necessary equipment control and
operating devices, such as pressure gauges, amp meters, volt meters, flow-rate indicators, temperature
gauges, CEMS, etc., shall be installed and operated properly and easily accessible to compliance
inspectors. A copy of all the manufacturers' and company's own subsequent operating instructions
for pollution control equipment and pollution emitting equipment shall be kept on site. These
instructions shall be available to all employees who operate the equipment and shall be made
available to compliance inspectors upon their request. Maintenance records shall be made available
to the executive secretary or executive secretary's representative upon request and the records shall
include the two-year period prior to the date of the request.
Status: In compliance. Shriners Hospital maintains records of maintenance activities conducted on
approved equipment for a minimum of two years.
12. The owner/operator shall comply with R307-1-4.7, UAC. This rule addresses unavoidable
breakdown reporting requirements. Any breakdown lasting longer than two hours shall be reported to
the executive secretary within three hours of the breakdown if reasonable, but in no case longer than
18 hours after the beginning of the breakdown. During times other than normal office hours,
breakdowns for any period longer than two hours shall be initially reported to the Environmental
Health Emergency Response Coordinator. Within seven calendar days of the beginning of any
breakdown lasting longer than two hours, a written report shall be submitted to the executive
secretary. The owner/operator shall calculate/estimate the excess emissions (amount above AO
limits) whenever a breakdown occurs. The total of excess emissions per calendar year shall be
reported to the executive secretary with the inventory submittal, as directed by the executive
secretary. The owner or operator of an installation suffering an unavoidable breakdown shall assure
that emission limitations and visible emission limitations are exceeded for only as short a period of
time as reasonable. The owner or operator shall take all reasonable measures which may include but
are not limited to the immediate curtailment of production, operations, or activities at all installations
of the source if necessary to limit the total aggregate emissions from the source to no greater than the
aggregate allowable emissions averaged over the periods provided in the source's approval order.
Status: In compliance. There were no reportable breakdowns at Shriners Hospital since the last
inspection.
FEDERAL REQUIREMENTS
In addition to the requirements of this AO, and applicable Area Source Rules, the following federal
programs have been found to apply to the following equipment at this facility.

40 CFR Part 60 Subpart Dc Standards of Performance for Small Industrial-Commercial-Institutional
Steam Generating Units
In compliance. Shriners Hospital operates two dual-fueled Boilers #1 and #2 are rated 8.03 NIMBTU,
were manufactured in 1994. Boiler #3 in AO Condition 5B is currently out of service. The rolling 12-
month consumption totals of natural gas from January to December 2021 indicated 7.8 x 106 standard
cubic feet. The diesel usage indicated 67 gallons for Boiler #1 and 70 gallons for Boiler #2.
40 CFR Part 63 Subpart ZZZZ — National Emission Standards for Hazardous Air Pollutants for Stationary
Reciprocating Internal Combustion Engines.
In compliance. Shriners Hospital operates a diesel-fiteled Emergency Generator, Caterpillar 3508
rated at 1,332 hp, and equipped with a non-resettable meter. This Emergency Generator qualifies as
an existing Stationary Engine >500 HP Located at Area Sources of HAP, constructed before June 12,
2006, and consequently, no notification, no initial compliance test, no operating limitations, and no
performance tests required. The rolling 12-month operation hours from January-December 2021,
indicated 12.32 hours. According to Sevier Valley Oil, the diesel is classified as Ultra Low Sulfur with
a sulfur content less than 0.0015 sulfur by weight. See the attachment for additional information.
R307 SOURCE RULES EVALUATION
UAC R307-203. Emission Standards: Sulfur Content of Fuels
In compliance. According to Sevier Valley Oil, the diesel is classified as Ultra Low Sulfur with a
sugar content less than 0.0015 sulfur by weight. See the attachment for additional information.
EMISSION INVENTORY: Shriners Hospital is currently not required to submit an emissions
inventory. The potentials to emit from the AO are (Pollutant, Tons/yr):
PM10, 0.49; SO2, 2.12; NO, 6.66; CO, 0.91; VOCs, 0.30

ENFORCEMENT ACTIONS: No enforcement actions within the past five years.
COMPLIANCE STATUS &
RECOMMENDATIONS: Shriners Hospital is in compliance with the conditions of Approval Order
DAQE-649-98 dated October 1, 1998.
