Hi Christine,
Could you peer review a name change for site 12157? It's an ownership change from "Buzzards Bench, LLC" to "Urban
Oil and Gas Group, LLC". This is the first out of eight sites with the same ownership change which will be included in the
final Name Change Letter. Hopefully I've successfully incorporated the edits you gave me for the Oshkosh Aerotech name
change here as well.

I have some suggested edits since we want to generate one letter for all the AOs they are requesting to change. I have
also included a similar project I did a few years ago to help. Please let me know what you think.
Once the letter is approved, you can have Teri go make the changes in Tempo on the back end.

Hi Christine,
Thanks for your edits! I ended up listing the AOs in an Appendix at the bottom of the letter because it worked better with
the spacing. Let me know if that's alright. An updated letter is attached below.
Last week I began projects for the rest of the "Urban Oil and Gas Group" name changes. Alan said that because projects
had already been started Teri wouldn't be able to update the other AOs on the back end. In the future I'll know to work on
a single name change and ask Teri to do the rest after it's approved but for now can I complete the remainder of the name
changes manually and send them to you for peer review? Let me know.

Hey Lucia,
Great job. I like your idea, but I would prefer to keep the same format (Table on first page) for consistency. It is better if it
matches other DAQ name change letters. If the entire table cannot fit on the first page, reduce the font slightly. Do not
worry about spacing - fix it up as best you can and Jeree can clean it up once she processes it for signature. You can
definitely reach out to her first in case there is something you don't want her to miss.
Regarding the several projects, that is fine, but I would just have this letter be attached to all of them. Then make sure you
go into each application set and do what you've done for the completed site.

I like them. I added a period to both LLCs and changed "above referred" to "applicable." I didn't change "has" to "have"
because I think it's referring to the singular "holder or party" rather than the plural AOs. I'm happy to change "has" to

realise one source was regulated as an exempted small source. Please see my suggested edits. Once those are made, I

- Send it to Jeree to process for signature. Let her know that once its approved, it will have to be attached to the sites
listed in the letter

I'm processing the name change request for Urban Oil and Gas with the UT Division of Air Quality and I want to clarify a
few parts of your application.
I received two Ownership Change Forms. One lists the current company name as "UOG CV, LLC" while the other lists it
as "Urban Oil and Gas Group, LLC." Which form is correct?
