The Utah Division of Air Quality (DAQ) has received your request on June 11, 2024, for a name and
ownership change for the holder of the above-referenced Approval Order (AO). According to your
request, we have noted in our records that the holder and party responsible for complying with the terms
and conditions contained in the above-referenced AO has been changed to ‘Oshkosh Aerotech’. This
change took effect on the date of this letter.

As authorized by the Utah Legislature, the fee for issuing this name change is a one-time filing fee in
addition to the actual time spent by the review engineer and all other staff on the project. Payment should
be sent to DAQ upon receipt of the invoice.

Hi Christine,
Could you peer review the name change I did for site 10925? I shared a google folder with all the relevant documents in it
and a copy of the name change letter is attached below.
There are two Application Sets under this name change. The one dated 6/24 has been updated. TEMPO won't let me
delete the Application set from 6/20. I wasn't sure what to do about it and Teri wasn't able to get rid of it either so I just left
it empty.
Let me know if you need anything else. If you have any feedback or advice I would love to hear it!

Great job with the letter. Please see my comments and let me know what you think. Additionally, I had some comments
about the Tempo file.
Under the Cover Page tab in Tempo:
1. In the Application Set in Tempo that you edited, the Relevant Documents should be the same as the title of the letter:
Notification of Company Name and Ownership Change from "JBT Corporation" to "Oshkosh Aerotech" for Approval Order
DAQE-AN109250011-18, dated September 19, 2018"
2. Delete the text in the Description of Additions
3. Under the Permit History section, you need to change "supercedes" to "incorporates". The AO number itself is not
changing as a result of this name change.
Under the Agency Interest Definition tab in Tempo.
1. Under the General Description > Alternate/Historic AI identifiers, the JBT Corporation needs an end date. The end date
should be the date of the application. The start date can be whenever they became the owner/operator of the current AO.

If I plug "Notification of Company Name and Ownership Change from "JBT Corporation" to "Oshkosh Aerotech" for

I was a little confused when TEMPO printed an email that didn't match the name in the header. I think it pulls the
name from the "primary AO contact" and the email from the "submits NOIs" contact. Should I reassign the Related
People so that the "primary AO contact" is the same as the "submits NOIs" contact"?

This is a common glitch and has happened to me every time I've done a name change. You just need to make the edits
once the document is generated.
It seems like TEMPO automatically prints the first part but ends with "for" instead of "from." Is there a way to get around

You make the edit once the document is generated. I am referring to the document name. I can elaborate tomorrow if you
have time before the trainings.

Oshkosh Aerotech wants Dan Pohly's name and contact listed on the Name Change Letter. However, when I tried to

This is an okay solution. Another way to to simply "unlink" RJ, then re-link him as having an "alternative AO contact"
relationship, instead of "submits NOIs for".

Please let me know when the WAL is ready for me. Once I have signed off on the project, you can proceed with sending
