RE: Request to Update Responsible Official and Facility Contact

Dear Mr. Humphreys:

On behalf AT&T Communications of the Mountain States, Inc. (all referred to herein as
“AT&T”), we are submitting this letter to request an update to the listed responsible
official (RO) on certain permits/registrations. AT&T is requesting that the previous RO,
Jeramiah German, be replaced by Travis Bawcum effective September 10, 2024 as Mr.
German has moved into another role within AT&T. As an Assistant Secretary, Mr.
Bawcum is a duly authorized representative for AT&T. Please note that AT&T is not
requesting changes to any RO other than Jeramiah German and that the address is the
same for both individuals.

In addition to the RO change request above, AT&T is requesting an update to the
facility contact name due to additional personnel changes. Please ensure the contact
is updated to the following individual:
