Dear Craig Christoffersen:

The Division is moving forward with the Operating Permit process as part of the SRF funding
process. The Division issued Plan Approval for this project on March 21, 2024.

Our understanding of the project was the construction of an 8-inch diameter PVC C900 water line,
including fire hydrants and other improvements, extending Ukon Water Company’s system to
connect and provide drinking water to the M & J Trailer Home Community (UTAH02078) water
system. The water line was installed along 450 W (4400 W) in Fielding, Utah running north to
18250 N.

We have determined that all conditions for operating permit issuance have been met. On this
basis, an Operating Permit for M & J Trailer Park Water Line is hereby issued as
constituted by this letter. You may now place M & J Trailer Park Water Line in service in your
water system.

Please maintain a copy of this letter with your permanent records for future reference.
