Dear Wendle Roberts:The Division of Drinking Water (the Division) received your request for the Operating Permit for the 2020 Edmunds Well REPL-1 from your consultant on May 3, 2023. The Division was unable to process the operating permit at that time due to the need for a new turbidity sample from the well and final source protection documentation was needed. The new turbidity sample was received from your consultant on March 4, 2025, and the source protection documentation was received on April 9, 2025. The Division issued Plan Approval for drilling this well on April 20, 2020, and Plan Approval for equipping this well on May 11, 2023. The DEQ Central District Engineer conducted an onsite final inspection of this facility on July 3, 2023. On April 9, 2025, the Division received your request for an exception to Rule R309-600-13(2)(d) regarding Land Use Agreements, for 2010 Edmunds Well REPL-1. According to this rule, the PWS (Public Water System) shall not introduce a new source into its system until copies of all applicable recorded land use agreements are submitted to the Division. Our understanding of the project is that it consisted of drilling a replacement well within approximately 15 feet of the original 2010 Edmunds Well. The original well has been abandoned according to the Utah Division of Water Right requirements as part of this project. The replacement well has been equipped with the existing submersible well pump and column piping from theabandoned 2010 Edmunds Well. The replacement well is also using the existing well house and welldischarge piping appurtenances. The replacementwell has a 10-inch diameter permanent steel casing and was drilled to a depth of 400 ft. The new well was also developed, and test pumped for over 24 hours at 100 gallons per minute (gpm). The submersible pump in this well has a 25 HP motor and produces 110 gpm. The 2010 Edmunds Well REPL-1 is identified as WS005 in the Division’s database. We have received the following information for the 2010 Edmunds Well REPL-1: Design engineer’s statement of conformance with approval conditions. As-built or record drawings. Recorded land use agreements and a temporary exception for land use agreements. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Additional turbidity sample showing compliance with turbidity MCL. We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for the 2010 Edmunds Well REPL-1 is hereby issued as constituted by this letter. You may now place the 2010 Edmunds Well REPL-1 (WS005)in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. The equipped well pump capacity of 2010 Edmunds Well REPL-1 is 110 gallons per minute (gpm). The safe yield of 2010 Edmunds Well REPL-1 is rated at 67 gpm, which is calculated based on two-thirds of the constant-rate aquifer drawdown test results at 100 gpm. The safe yield of 67 gpm is the basis for determining the maximum number of connections that 2010 Edmunds Well REPL-1 can serve.Temporary Exception to R309-600-13(2)(d) The background and basis of your exception request to R309-600-13(2)(d) is included in the attached Temporary Exception to Rule - Approval Form. On the basis of the submitted documents, an exception to R309-600-13(2)(d), Land Use Agreements, is hereby granted for 2010 Edmunds Well REPL-1 (WS005). Issuance of this Operating Permit changes this water system’s monitoring and reporting requirements. The monitoring requirements for this well are given below.
