SECTION I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401] I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions covered by this AO must be reviewed and approved. [R307-401-1]
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by the owner/operator, shall be made available to the Director or Director's representative upon request, and the records shall include the two-year period prior to the date of the request. Unless otherwise specified in this AO or in other applicable state and federal rules, records shall be kept for a minimum of two (2) years. [R307-401-8] I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall, to the extent practicable, maintain and operate any equipment approved under this AO, including associated air pollution control equipment, in a manner consistent with good air pollution control practice for minimizing emissions. Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. All maintenance performed on equipment authorized by this AO shall be recorded. [R307-401-4]

II.B.1.a The owner/operator shall not allow visible emissions from any point or fugitive emission source associated with the installation or control facilities to exceed 10% opacity. [R307-401-8] II.B.1.a.1 Opacity observations of emissions from stationary sources shall be conducted in accordance with 40 CFR 60, Appendix A, Method 9. [R307-401-8]
II.B.2 Paint Booths II.B.2.a The owner/operator shall equip each paint booth with Andrea exhaust filter media made of non-fire-supporting paper paint arrestor filters or equivalent to control particulate emissions. The owner/operator shall route air exiting the paint booths through this control system before venting to the atmosphere. [R307-401-8]

II.B.2.b The owner/operator shall comply with the applicable requirements and limitations in R307-350 Miscellaneous Metal Parts and Products Coatings. [R307-350] II.B.3 VOC and HAP Limitations
II.B.3.a The owner/operator shall not emit more than the following from evaporative sources (painting, printing, coating, and/or cleaning and associated operations) on site: 10.00 tons per rolling 12-month period for VOCs, including all HAPs below 254 pounds per rolling 12-month period for ethyl benzene 26 pounds per rolling 12-month period for formaldehyde 173 pounds per rolling 12-month period for methanol 101 pounds per rolling 12-month period for methyl isobutyl ketone 965 pounds per rolling 12-month period for toluene 1421 pounds per rolling 12-month period for xylene 1.47 tons per rolling 12-month period for total HAPs. [R307-401-8] II.B.3.a.1 The owner/operator shall calculate a new 12-month total by the 20th day of each month using
data from the previous 12 months. The owner/operator shall use a mass-balance method to
calculate emissions from evaporative sources. The owner/operator may use the following equations with applicable units to comply with the mass-balance method:
VOCs = [% VOCs by Weight/100] x [Density] x [Volume Consumed]
HAP = [% HAP by Weight/100] x [Density] x [Volume Consumed].

II.B.3.a.2 The owner/operator shall use a mass-balance method to quantify any amount of VOCs and HAPs reclaimed. The owner/operator shall subtract the amount of VOCs and HAPs reclaimed from the quantities calculated above to provide the monthly total emissions of VOCs and HAPs. [R307-401-8]

II.B.3.a.3 The owner/operator shall keep records each month of the following: A. Name of the VOC- and HAP-emitting material, such as paint, adhesive, solvent, thinner, reducers, etc. B. Density of each material used (pounds per gallon) C. Percent by weight of all VOC and HAP in each material used D. Volume of each VOC- and HAP-emitting material used E. The amount of VOC and HAP emitted monthly by each material F. The amount of VOC or HAP emitted monthly from all materials used. G. The amount of VOCs or HAPs reclaimed for the month H. The total amount of VOCs, the total amount of each HAP, and the total amount of all HAPs combined emitted from all materials. [R307-401-8]
II.B.4 Conditions on the Sandblasting Operations
II.B.4.a The owner/operator shall control process streams from the sandblasting operations with a baghouse. The owner/operator shall route all exhaust air from the sandblasting operations through the baghouse before venting to the atmosphere. [R307-401-8]
II.B.4.b The owner/operator shall not exceed 3,000 hours of operations per rolling 12-month period of sandblasting operations. [R307-401-8]
II.B.4.b.1 The owner/operator shall: A. Determine hours of operations by supervisor monitoring and maintaining of an operations log B. Calculate a new 12-month total by the 20th day of each month using data from the previous 12 months C. Record hours of operation on a daily basis D. Keep records of hours of operations for all periods when the plant is in operation. [R307-401-8]
II.B.4.c The owner/operator shall comply with all applicable requirements of R307-306 PM10

concerns with the review of the draft permit conditions. Upon resolution of your concerns, please email Katie Andersen at kandersen@utah.gov the signed cover letter. Upon receipt of the signed cover letter, the DAQ will prepare an Approval Order (AO) for signature by the DAQ Director. If Flowserve Corporation does not respond to this letter within 10 business days, the project will move forward without source concurrence. If Flowserve Corporation has concerns that cannot be resolved and

Hello Brian Pike,
The Utah Division of Air Quality (DAQ) has initiated a 10-Year Review for the purpose of updating the language, format,
and layout of Approval Order DAQE AN-108370002-14 for Flowserve Corporation Automatic Control Valves and Sub-
component Manufacturer Plant. The DAQ typically initiates 10-Year Reviews once an approval order has been active
for 10 or more years.
I've been assigned as the project engineer for this permit update. I have some questions to verify contact information,
location information, and equipment information. Based on the latest compliance inspection report, you are listed as the
source contact.
Would you be the best person to direct my questions to or is there another person over the permitting or environmental
concerns I could talk with?

2. The current business name is Flowserve Corporation. Is this correct?

5. The current Approval Order lists the following equipment: two (2) natural gas-fired ovens rated at 0.75 MMBtu/hr each,
three (3) paint booths, miscellaneous anodizing equipment, sandblasting booth and baghouse, and a welding booth with
miscellaneous welding equipment. Is this equipment still in use? Are there any other pieces of equipment that have not
been added to the permit?
6. Please provide the OSHA Standard Industrial Classification code (4 digits) for the facility.
At the moment, I believe this is all the information I need to update the current Approval Order.
Before the updated Approval Order is finalized, you will have an opportunity to review a draft version and verify that the
information in the draft is correct.
