The Division of Air Quality (DAQ) received your request to revoke Approval Order (AO)
DAQE-AN0142430002-11 on February 10, 2025. TM Crushing, LLC requested this AO be revoked due
to the site being developed into a subdivision and the aggregate facility permanently ceasing operations.
The DAQ has revoked the above-referenced AO effective as of the date of this letter.
As authorized by the Utah Legislature, the fee for revoking an AO is the actual time spent by the review
engineer and all other staff on the project. Payment should be made to the DAQ upon receipt of the
invoice.

Aggregate pit. A jaw crusher, cone crusher, screen, generator and
miscellaneous mobile equipment are located on site. Sources of
emissions include those from the crushing and screening process, haul
roads, loader traffic, stockpile and disturbed area wind erosion and fuel
combustion.
Primary Crushing process: This operation involves extraction,
processing, conveying and stockpiling. Material is delivered to a crusher
by front-end loader. One or more conveyors or belt feeders transfer the
crushed material to a triple deck screen that classifies the material by
size. Material from the screen goes to different stockpiles via conveyors
and/or stackers. Material that cannot pass through the first set of screens
is transferred to a secondary crusher.
Secondary Crushing process: The secondary crushing process is actually
a sub-process of the primary. Material that does not pass through the
triple deck screen can be sent to the secondary crusher for further size
reduction. The crushed material is transferred by conveyors back to the
triple deck screen for size classification. Secondary crushing is an
optional step and may be added or removed as necessary.
Screening process: Screening is the process of classifying material that
has been crushed. In most cases, the screen unit is comprised of three
decks, each with a different mesh screen. Material comes from the
crusher and is classified by the screen.

Abstract
TM Crushing, LLC (TM Crushing) has submitted a Notice of Intent to operate a new aggregate
processing plant in West Jordan in Salt Lake County. Annual production is limited to 250,000 tons of
processed aggregate. Equipment includes two crushers, a screen, conveyors/stackers, and a generator.
Salt Lake County is a nonattainment area of the NAAQS for PM10, PM2.5 and SO2, and a maintenance
area for CO (Salt Lake City only) and ozone. NSPS and MACT regulations apply to this source. Title V
of the 1990 Clean Air Act applies to this source.
A dispersion modeling analysis was performed for the source, and the predicted total NO2 concentrations
are less than the NAAQS.
The emissions, in tons per year, will be as follows: PM10 9.80, PM2.5 8.57, NOx 14.19, SO2 0.51, CO 7.26,
VOC 0.89 and total HAPs 0.01.
This air quality AO authorizes the project with the following conditions and failure to comply with any of
the conditions may constitute a violation of this order. This AO is issued to, and applies to the following:

Section I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in
the UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions
refer to those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401]
I.3 Modifications to the equipment or processes approved by this AO that could affect the
emissions covered by this AO must be reviewed and approved. [R307-401-1]
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by
the owner/operator, shall be made available to the Executive Secretary or Executive Secretary's
representative upon request, and the records shall include the two-year period prior to the date of
the request. Unless otherwise specified in this AO or in other applicable state and federal rules,
records shall be kept for a minimum of two (2) years. [R307-401-8]
I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators
shall, to the extent practicable, maintain and operate any equipment approved under this AO,
including associated air pollution control equipment, in a manner consistent with good air
pollution control practice for minimizing emissions. Determination of whether acceptable
operating and maintenance procedures are being used will be based on information available to
the Executive Secretary which may include, but is not limited to, monitoring results, opacity

observations, review of operating and maintenance procedures, and inspection of the source.
All maintenance performed on equipment authorized by this AO shall be recorded.

I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Unavoidable
Breakdowns. [R307-107]
I.7 The owner/operator shall comply with UAC R307-150 Series. Inventories, Testing and
Monitoring. [R307-150]
Section II: SPECIAL PROVISIONS
II.A The approved installations shall consist of the following equipment:
II.A.1 West Jordan Aggregate Processing Facility
II.A.2 Primary Crusher
One (1) crusher rated at 600 tons per hour (tph); manufactured in 1977
II.A.3 Secondary Crusher
One (1) crusher rated at 350 tph; manufactured in 1998
II.A.4 Screen
One (1) screen rated at 300 tph; manufactured in 2002
II.A.5 Generator
One (1) diesel-fired generator rated at 700 kW; manufactured after 4/1/2006
II.A.6 Conveyors and Stackers
Eight (8) conveyors and stackers
II.B Requirements and Limitations
II.B.1 Site-wide Requirements and Limitations
II.B.1.a TM Crushing shall notify the Executive Secretary in writing when the installation of the
equipment listed in Condition II.A has been completed and is operational. To ensure proper
credit when notifying the Executive Secretary, send your correspondence to the Executive
Secretary, attn: Compliance Section. [R307-401-18]
II.B.1.a.1 If the construction and/or installation have not been completed within 18 months from the date
of this AO, the Executive Secretary shall be notified in writing on the status of the
construction and/or installation. At that time, the Executive Secretary shall require
documentation of the continuous construction and/or installation of the operation and may
revoke the AO. [R307-401-18]

II.B.1.b The following production and consumption limits shall not be exceeded:
A. 250,000 tons of aggregate processed per rolling 12-month period
B. 2,700 hours of operation per rolling 12-month period for the diesel-fired generator.

II.B.1.b.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a
new 12-month total by the twentieth day of each month using data from the previous 12
months. Records of production shall be kept for all periods when the plant is in operation.
Production shall be determined by the belt scale records near the initial feeder, or any other
methods approved by the Executive Secretary. Production records shall be maintained using
an operations log. Hours of operation shall be determined by supervisor monitoring and
maintaining of an operations log. The records of production and hours of operation shall be
kept on a daily basis. [R307-401-8]
II.B.1.c Visible emissions from the following emission points shall not exceed the following values:
A. All crushers - 15% opacity
B. All screens - 10% opacity
C. All conveyor transfer points - 10% opacity
D. All conveyor drop points - 20% opacity
E. All other points - 20% opacity

II.B.1.c.1 Opacity observations of emissions from stationary sources shall be conducted according to 40
CFR 60, Appendix A, Method 9.
For equipment subject to NSPS, opacity shall be determined by conducting observations in
accordance with 40 CFR 60.11(b) and 40 CFR 60, Appendix A, Method 9.
Initial visible emission observations shall be conducted according to 40 CFR 60.11 and 40
CFR 60.675. A certified observer must be used for these observations. Emission points which
are subject to the initial observations are those as defined in 40 CFR60.670. [R307-401-8]
II.B.2 Conditions on Fugitive Dust Emissions
II.B.2.a TM Crushing shall comply with an FDCP acceptable to the Executive Secretary for control of
all dust sources associated with the aggregate processing operations. TM Crushing shall
submit the FDCP to the Executive Secretary, attention: Compliance Section, for approval
within 30 days of the date of this AO. [R307-309]
II.B.2.b The unpaved haul road length shall not exceed 0.25 miles.
The owner/operator shall use chemical suppressant and/or water to control fugitive dust
emissions from the above haul road. Application frequencies for chemical treatment and/or
water spray shall follow the requirements in the FDCP. [R307-401-8]

II.B.2.b.1 The owner/operator shall cover all unpaved haul roads and wheeled-vehicle operational areas
with road base material and shall use water and/or chemical suppressant applications to
maintain opacity limits listed in this AO. If the temperature is below freezing, the
owner/operator may stop applying water to the unpaved haul roads and wheeled-vehicle
operational areas.
Records of water application shall be kept for all periods when the plant is in operation. The
records shall include the following items:
A. Date and time treatments were made
B. Number of treatments made and quantity of water applied
C. Rainfall amount received, if any
D. Records of temperature, if the temperature was below freezing.

II.B.2.b.2 Visible fugitive dust emissions from unpaved haul-road traffic and mobile equipment in
unpaved operational areas shall not exceed 20% opacity at any point. Visible emission
determinations shall use procedures similar to Method 9. The normal requirement for
observations to be made at 15-second intervals over a six-minute period, however, shall not
apply. Visible emissions shall be measured at the densest point of the plume but at a point not
less than 1/2 vehicle length behind the vehicle and not less than 1/2 the height of the vehicle.

II.B.2.c Water sprays or chemical dust suppression sprays shall be installed at the following points to
control fugitive dust emissions:
A. All crushers
B. All screens
C. All conveyor transfer points
The sprays shall operate to meet the opacity limits or as determined by the Executive
Secretary. [R307-401-8]
II.B.2.d The storage piles shall be watered to maintain opacity limit listed in this AO. Records of
water and/or chemical treatment shall be kept for all periods when the plant is in operation.
Records of water and/or chemical treatment shall be made available to the Executive Secretary
or Executive Secretary's representative upon request. [R307-401-8]
II.B.3 Conditions on the Diesel-Fired Generator
II.B.3.a The owner/operator shall use #1 and/or #2 fuel oil or any combination of both as a fuel in the
diesel-fired generator. [R307-401-8]
II.B.3.a.1 The sulfur content of any fuel oil or diesel burned shall not exceed 15 ppm by weight.

II.B.3.a.2 The sulfur content shall be determined by ASTM Method D-4294-89 or approved equivalent.
Certification of the fuel oil shall be either by the source's own testing or test reports from the
fuel marketer. The sulfur content certification or the test reports shall be available on-site for
each load delivered. [R307-401-8]
II.B.3.b Visible emissions from the diesel-fired generator shall not exceed 20% opacity. [R307-401-8]
II.B.3.b.1 Opacity observations of emissions from stationary sources shall be conducted according to 40
CFR 60, Appendix A, Method 9. [R307-401-8]

When I look up the "7604 South Highway 111 West Jordan" on google earth it doesn't match the coordinates you provided for the site. Do you know why that might
be? Is there a better address available?

The original address used in this permit is incorrect. Our Pit was located 7800 south Bacchus highway on the northwest corner. There is some
house there now.
