The Division of Environmental Response and Remediation (DERR) requested that you submit a Subsurface Investigation Report (SIR). The WP&B was due on February 29th, 2024. To date, the
WP&B has not been received and is past due.
Please submit a WP&B to the DERR by (1 month). Failure to comply with this notice may result in issuance of a Notice of Non-Compliance or Order. The DERR has the authority to seek
civil penalties of up to $10,000 for each day of violation pursuant to Utah UST Act, Section 19-6-425.
