In compliance with the Approved Blending Plan for Well #2 (WS002) for Eagle Mountain City, I am
submitting this quarterly report. The report includes the attached spreadsheet showing the monthly
arsenic levels and the Weighted Annual Average. The current Rolling Annual Average, through the first
quarter, for 2025 is 0.0054 mg/L. I have also attached the Certificate of Analysis from Chemtech-Ford
Laboratories for each of the sample months included in this Quarterly Report.
The sampling will continue as outlined on the Approval Letter dated March 30, 2009 and Quarterly
