SOURCE INFORMATION
General Description
Mountain View Mushrooms (MVM)is a producer and wholesaler of fresh mushrooms located in
Fillmore, Millard County. MVM starts the process outdoors by creating compost for the spores to grow
on. When the compost is ready, it is added to growing trays and taken inside to a pasteurization room to
eliminate undesirable bacteria. Once the trays are prepared, the mushroom spawn is mixed with the
compost, and the growing tray is placed in another room for several weeks to allow the spawn to grow.
After spawning, the growing trays are placed in a growing room, and mushrooms are harvested several
weeks later. MVM harvests mushrooms every day to ship them to a variety of places in the Intermountain
West region.

Project Description
MVM is permitted to operate one (1) primary natural gas-fired, 600 hp boiler and one (1) standby
dual-fuel, 300 hp boiler. MVM is requesting to operate the 300 hp (12 MMBtu/hr) standby dual-fuel
boiler as the primary boiler and replace the primary 600 hp boiler with a standby, dual-fuel, 100 hp (4.18
MMBtu/hr) boiler. Both boilers operate with low-NOx burners. These equipment changes result in an
actual decrease of all criteria pollutant and HAP emissions.
Therefore, this project meets the requirements of UAC R307-401-12 Reduction in Air Pollutants and does
not require a public comment period.

SECTION I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401] I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions covered by this AO must be reviewed and approved. [R307-401-1]

I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by the owner/operator, shall be made available to the Director or Director's representative upon request, and the records shall include the two-year period prior to the date of the request. Unless otherwise specified in this AO or in other applicable state and federal rules, records shall be kept for a minimum of two (2) years. [R307-401-8] I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall, to the extent practicable, maintain and operate any equipment approved under this AO,
including associated air pollution control equipment, in a manner consistent with good air pollution control practice for minimizing emissions. Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to
the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. All maintenance performed on equipment authorized by this AO shall be recorded. [R307-401-4]
I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns. [R307-107]
I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150]

SECTION II: SPECIAL PROVISIONS II.B REQUIREMENTS AND LIMITATIONS II.B.1 The Mountain View Mushroom facility shall be subject to the following:
II.B.1.a The owner/operator shall maintain a copy of this AO posted on site. The AO shall be available to the employees who operate the air emission-producing equipment. These employees shall receive instruction as to their responsibilities in operating the equipment in compliance with all of the relevant conditions listed below. [R307-401-8] II.B.1.b The owner/operator shall not allow visible emissions from the boiler stacks or any other stationary source on site to exceed 10% opacity. [R307-401-8]

II.B.1.b.1 Opacity observations of emissions shall be conducted in accordance with 40 CFR 60, Appendix A, Method 9. [R307-401-8] II.B.1.c The owner/operator shall not consume more than 224,000,000 cubic feet of natural gas per rolling 12-month period. [R307-401-8]
II.B.1.c.1 The owner/operator shall: A. Determine fuel consumption by examination of fuel purchasing records B. Record consumption on a monthly basis C. Use the consumption data to calculate a new 12-month total by the last day of each month using data from the previous 12 months D. Keep the consumption records for all periods the plant is in operation. [R307-401-8] II.B.1.d The owner/operator shall not produce less than 38,200 pounds of steam per hour during all
compliance testing operations. [R307-401-8]
II.B.1.e The owner/operator shall use natural gas as a primary fuel in the boilers. Diesel fuel (#2 fuel oil) may only be used for emergencies. The owner/operator shall not operate the 300 hp (12 MMBtu/hr) boiler and 100 hp (4.18 MMBtu/hr) boiler for more than 48 hours each per rolling 12-month total during periodic testing, maintenance, or operator training on liquid fuel. There is no time limit on the use of the boilers during gas curtailment, gas supply interruption, or startups. [40 CFR 63 Subpart JJJJJJ, R307-401-8] II.B.1.e.1 The owner/operator shall:
A. Determine hours of operation by monitoring and maintaining an operations log
B. Record hours of operation and reason for operation on liquid fuel each day
C. Use the hours of operation to calculate a new rolling 12-month total by the 20th day
of each month using data from the previous 12 months
D. Keep hours of operation records for all periods the plant is in operation.

II.B.1.f The owner/operator shall only combust fuel that meets the definition of ultra-low sulfur diesel (ULSD), which has a sulfur content of 15 ppm or less. [R307-401-8]
II.B.1.f.1 To demonstrate compliance with the ULSD fuel requirement, the owner/operator shall maintain
records of diesel fuel purchase invoices or obtain certification of sulfur content from the diesel
fuel supplier. The diesel fuel purchase invoices shall indicate that the diesel fuel meets the ULSD

II.B REQUIREMENTS AND LIMITATIONS
II.B.1 Site-wide Requirements
II.B.1.a The owner/operator shall not exceed the following production and operational limits:
A. 300,000 tons of aggregate processed per rolling 12-month period
B. 65,000 tons of screened/washed sand per rolling 12-month period
C. 200 tons per hour of aggregate processed
D. 12 hours of operation per day between 6:00 AM and 10:00 PM.

II.B.1.a.1 To determine compliance with production/operational limits, the owner/operator shall: A. Determine production by supervisor monitoring and maintaining an operations log B. Calculate a new 12-month total by the 20th day of each month using data from the previous 12 months C. Keep records of production for all periods the plant is in operation D. Keep records of production on a daily basis. [R307-401-8]
II.B.1.b The owner/operator shall not exceed the opacity limits for the following points:
A. Crushers - 12 %
B. Screens - 7%
C. Conveyor transfer points - 7% opacity
D. Conveyor drop points - 20%
E. Diesel Engines - 20%
F. All other points - 20%.

II.B.1.b.1 Opacity emissions from stationary sources shall be conducted according to 40 CFR 60, Appendix A, Method 9. [R307-401-8]

II.B.1.c The owner/operator shall install water sprays or chemical dust suppression sprays at the emission points below to comply with the opacity limits in this AO: A. All Crushers B. All screens C. All conveyor transfer points. [R307-401-8]
II.B.1.d The owner/operator shall use #1 and/or #2 fuel oil as fuel in the generator engines. [R307-401-8]
II.B.1.e The owner/operator shall only combust diesel fuel that meets the definition of ultra-low sulfur diesel (ULSD) in the generator engines. ULSD has a sulfur content of 15 ppm or less. [40 CFR 60 Subpart ZZZZ]
II.B.2 Haul Road and Fugitive Dust Requirements
II.B.2.a The owner/operator shall comply with all applicable requirements of R307-309 for PM2.5 non-attainment areas for fugitive emission and fugitive dust sources. In addition to addressing all applicable requirements in R307-309, the owner/operator shall also include a plant to test the silt loading of paved surfaces in the fugitive dust control plan. [R307-309, R307-401-8] II.B.2.b The owner/operator shall not exceed two (2) miles of haul roads. [R307-401-8]
II.B.2.c The owner/operator shall not allow visible emissions from haul-road traffic and mobile equipment in operational areas to exceed 20% opacity on site and 10% opacity at the property boundary. [R307-309-5, R307-401-8]
II.B.2.c.1 Visible emission determinations for fugitive dust from haul roads and operational areas shall use procedures similar to Method 9. The normal requirement for observations to be made at
15-second intervals over a six-minute period, however, shall not apply. Visible emissions shall
be measured at the densest point of the plume but at a point not less than one-half vehicle length behind the vehicle and not less than one-half the height of the vehicle.

II.B.2.d The owner/operator shall use water spray and/or chemical treatment on unpaved operational areas used by mobile equipment such that the surface is in a damp/moist condition unless the temperature is below freezing. The Director shall approve a chemical treatment plan if any chemical treatment is used. [R307-401-8]
II.B.2.d.1 The owner/operator shall keep records of the following regarding water spray/chemical treatment
when the plant is in operation:
A. Date and time of treatment
B. Number of treatments made, dilution ratio, and quantity
C. Rainfall received, if any, and approximate amount
D. Time of day treatments were made.

II.B.2.f The owner/operator shall limit vehicle speed along the haul roads to 15 miles per hour. The owner/operator shall post vehicle speed on site at the start of the haul road such that it is clearly visible from the haul road. II.B.2.h The owner/operator shall use water sprays and/or chemical treatment to control fugitive emissions from storage piles and unpaved operational areas, to comply with the opacity limits in this AO. Records of water and/or chemical treatment shall be kept for all periods when the plant is in operation. [R307-401-8]
II.B.2.i The owner/operator shall not exceed 25 g/m2 of silt loading on paved surfaces. [R307-401-8]
II.B.2.i.1 The owner/operator shall test for silt loading once a month while the plant is in operation and shall follow the procedure in Appendix C of Compilation of Air Pollution Emission Factors (AP-42). [R307-401-8]
