Location: NA Date: 2025-03-21 08:51:04 Foxit PhantomPDF Version: 9.7.5 David F Booth Clive Facility Specific Work Instruction CL-SH-WI-107-03 Rev 2 Confined Space Process for Creating and Maintaining Specific Confined Space Entry Evaluations Non-Proprietary Page 2 of 5 1 Purpose 1.1 This work instruction is to document the site process for creating and maintaining permit required confined space entry evaluations for new or modified confined spaces. 2 Process for Permit Required Confined Space Entry Procedures 2.1 The facility will use (CL-SH-WI-107-F1) for both confined space entry procedures (ES-SH-PR-107-F2) and the Confined Space Evaluation (ES-SH-PR- 107-F1) for each listed confined space in the site inventory. 2.2 The Safety and Health Department, or designee will prepare a specific confined space entry evaluation for the space using CL-SH-WI-107-F1. 2.3 Entry evaluations will be reviewed annually by the Safety and Health Department or designee. Any corrections that are needed will be addressed and corrected during the review process. 2.4 Entry evaluations will be documented and kept by the Safety and Health Department. Evaluations will be available electronically for all employees. 2.5 The Entry Supervisor will review the entry evaluation prior to allowing entry into the confined space.
