Follow Up to Notification of Liquid Waste Spill with 9088-30-0215
Dear Mr. Hansen, In a letter dated March 4, 2025 (CD-2025-047), EnergySolutions notified the Director of the Utah
Division of Waste Management and Radiation Control (DWMRC) of a potential discrepancy from
two intermodals from Generator 9088-30-0215. The two intermodals (ESUU100585 and ESUU100454) Bates Number PR010067, arrived on February 13, 2025 and were being stored in the Restricted Area while they awaited the Deferred Chemical (DC Sample) analysis results. Follow up to that notification is provided herein.
The receipt inspection of Shipment 9088-30-0215 (Bates # PR10067) Intermodals ESUU100585 and ESUU100454 that arrived on February 13, 2025, did not indicate any leaks with these containers at the time of receipt. On March 4, 2025, Operations was preparing the containers for disposal, when the leaks were identified. Liquid was dripping from the bottom seal of Intermodal ESUU100454 (see
attached photograph) and from the floor of Intermodal ESUU100585 (see attached photograph). The
Contingency Plan was implemented, and the leaks were contained. Results from the onsite gamma spectroscopy analysis of the liquid detected 81.6 pCi/mL of Cesium-137 (see attached survey). The area was cleaned up and a radiological survey performed in the field
on and around the railcar (within the restricted area), yielded results less than Minimal Detectable
Activity (MDA) Alpha and 2000 Beta dpm/100cm2 (see attached survey). An additional Radiological (Post Spill) Survey will be performed on Railcar GIMX 516257 to release it from the Restricted Area.
EnergySolutions contacted the generator on March 4, 2025, to inform them of this discrepancy with
Intermodals ESUU100585 and ESUU100454. These intermodals were loaded by the generator at the same time as the previously identified leaking intermodal reported in CD-2025-030 and CD-2025-038. The generator responded to this discrepancy and immediately applied corrective actions for future shipments. This includes additional amounts of absorbent in each container, both granular and
mats, to mitigate the potential of free liquid generation during transport. Upon notification, the
generator contacted DMWRC and provided details of corrective actions. The generator is creating a formal Corrective Action Plan (CAP) to prevent reoccurrence of this issue. To aid in their corrective

actions, the generator also requested Clive operations assess the tightness of the door chains and provide pictures and notes as the back door on the intermodal is opened; they are looking for any deformities or obstructions in the seals that may have caused this issue and can then tailor corrective
actions to ensure this does not occur in the future.
Condition Report # CR-2025-0178 has been written to track resolution of this issue. Please consider this the follow-up written notification (required in Radioactive Materials License
UT2300249, Condition 68) for this discrepancy.
