Dear Mr. Tao:
Terracon Consultants, Inc. (Terracon), on behalf of Hansen & Hansen, LLC. and Conrad &
Bischoff, Inc. has prepared the attached Work Plan and Budget (WP&B) JUR-14 to develop a
Corrective Action Plan (CAP) for the above-referenced site. Based on the investigation
activities conducted at the site in 2020 through 2023, additional corrective action is required
to address petroleum impacts at the site. The attached WP&B JUR-14 was prepared in
response to your request to address Corrective Action Plan Requirements, dated May 9,
2024 and the WP&B submission extension granted until May 22, 2024, via your email, dated
May 22, 2024.

1.0 Project and Background Information
The property is an active gas station, located at 620 West 200 North, in Logan, Utah
(hereinafter, referred to as the site). A Leaking Underground Storage Tank (LUST) release was
identified at the site in 1996 (LUST ID JUR). Assessment activities were conducted; however, no
remediation was conducted. The JUR release file was closed in 2012 with residual concentrations
of petroleum hydrocarbons in groundwater above the Division of Environmental Response and
Remediation (DERR) Tier 1 Screening Criteria. Petroleum impacts were identified at the site in
2020 during due diligence investigation activities and additional soil, groundwater, and high-
resolution site characterization (HRSC) investigations were conducted in 2020 through 2023. A
potential source area was identified approximately in the northwest portion of the dispenser area
during the HRSC investigation; however, we understand no damaged, loose, or leaking
components were identified during the subsequent observations of the accessible portions of the
dispensers and piping, and tightness tests conducted on the underground storage tank system
have passed the applicable criteria.
Mr. Hong-Lei Tao, of DERR, requested a Work Plan and Budget (WP&B) for preparation of a
Corrective Action Plan (CAP) in a letter, dated May 9, 2024. Mr. Tao subsequently granted an
extension for the WP&B until June 15, 2024, in an email sent to Ms. Jill Hernandez, dated May
22, 2024.
2.0 Scope of Services
The proposed Scope of Services was prepared in accordance with DERR’s request.
Task 1 Preparation of a WP&B
Terracon generated this WP&B to develop a CAP that will address the next assessment and
remedial requirements at the site. Prior to development of the CAP, a meeting will be conducted
with the Hansen & Hansen, LLC., Parkland USA Corporation, Terracon, and DERR
representatives, as appropriate.

Task 2 Preparation of a Corrective Action Plan
Pending your approval of the WP&B, a Terracon subject matter expert and the project reviewer
will be consulted regarding assessment and remedial options. Terracon will collaborate with
Hansen & Hansen, LLC, Parkland USA Corporation, and DERR, as appropriate, to develop a CAP
to address:

■ additional site characterization, if necessary, to assess the source of petroleum impacts at
the site
■ to assess remedial options and requirements to protect human health and the
environment
The next remediation steps will be evaluated based on their feasibility, cost effectiveness, and
applicable cleanup technologies.
The CAP will be submitted to UDEQ for review by July 8, 2024. Details of the estimated cost
required for preparation of WP&B JUR-14 and CAP are provided on the attached Budget Estimate
and summarized, as follows:

Terracon will prepare and submit a Petroleum Storage Tank Trust Fund Reimbursement Claim to
include eligible fees for the WP&B and CAP development for reimbursement to the responsible
