Hong-Lei,
A Correcve Acon Plan (CAP) and associated Work Plan and Budget (WP&B) were requested in the aached UDEQ leer addressed to Mr. James D. Hansen,
dated May 9, 2024. An extension was granted in your email, dated May 22, 2024. The inial version of WP&B JUR-14 aached was submied in an email,
dated June 14, 2024, and included development of the WP&B and preparaon of a CAP. Approval was not received for the June 14, 2024 version of WP&B
JUR-14, which included addional site characterizaon, if necessary, and development of a CAP. A revised WP&B JUR-14 was requested during a discussion
with you, me, and Mike Pecorelli at the UDEQ office on June 20, 2024. The revised WP&B JUR-14 was approved, which included the following:
Labor to prepare the revised WP&B
Labor for research and discussion regarding the ghtness tesng documents and revision of the PST fund agreement
Assessing remedial opons
A remedial approach coordinaon meeng and summary leer of the proposed approach

Based on our discussion during the remedial opons meeng on March 27, 2025, we are requesng an amendment to the approved JUR-14 funds to include
$1,506 for preparaon of the inial WP&B JUR-14, which was submied on June 14, 2024. Please let me know if the inial WP&B JUR-14 preparaon labor
can be submied for reimbursement.
