Subject: UDOT Loa Station MS4 Improvements Approval to Use
Dear Mr. Thornock:
The Division of Water Quality (“Division”) issued a Construction Permit for theMS4 Improvements for the UDOT Loa Station on June 15, 2022. Due to lack of funding the construction was
not completed at that time. On February 13, 2024 the Division received plans and specifications for the project that included additional construction items concerning storm water and
the sander racks. Due to fiscal year funding UDOT will break the project into Phase I (constructed in 2024) and Phase II (estimated to be constructed in 2025). The final updated plans
and specifications for Phase I were received by the Division on April 17, 2024 and the Construction Permit was issued on May 13, 2024.
Inspections were completed on August 1, 2024 and August 8, 2024. On the inspection on August 8th a rip in the primary liner was observed. Mike Johnson (Hales Sand and Gravel) indicated
that the liner was snagged by the skid steer. The rip was repaired with a patch using a heat gun and rollers. The patch was then tested with an air lance test at 60 psi. This is approved.
The work has now been completed and a final inspection was completed on February 28, 2025. With the help of Donna Beagley, I was recently provided with access to ProjectWise and am
now able to look at all the testing and inspections for this project.
Based on the final inspection and the review of theconstruction documentation in ProjectWise, DWQ approves the use of theUDOT Loa StationMS4 Improvements.
