60.1 13b(a)(2) Internal Floating Roof Inspection
As required by 40 CFR Subpart Kb $60.115b(a)(3), the Tesoro Refining & Marketing Company, LLC. Salt
Lake City Refinery reports that during the October 17,2024 l0-year inspection of Tank 188, two sections of
the tank's primary seal was observed to be detached and accumulated product was observed on the floating roof
around one roof leg and one vacuum breaker. The inspection was conducted while the tank was in service.
The attached inspection report (see Attachment A) contains the date of measurement, and inspection described
in $60.113b(aX2) and (a)(4). Subsequent repairs have been made while the tank remained in service, with a
passing inspection completed on November 11,2024. The November I l, 2024 inspection report is also
included as part of Attachment A.
This notification is being forwarded to your office with the understanding the Division has been delegated
authority to implement and enforce standards of performance for new stationary sources located in Utah. If
