This Temporary Operating Permit Expires on April 30, 2026.
Dear Caleb Lee Patton:The Division of Drinking Water (the Division) received a request for a Temporary Operating Permit (TOP) for the 5,000-Gallon Tank from your water system operator,
Yvette Nielson, on April 2, 2025. The Division issued Plan Approval for this project on November 3, 2020. The DEQ Central District Engineer conducted an onsite final inspection of this
facility on March 12, 2025.
Our understanding of the project is that it included the 5,000-Gallon Concrete Tank (ST002) as well as new arsenic removal filters and redundant booster pumps to provide pressure to
the existing hydropneumatic tank. The arsenic filtration equipment has not performed adequately and therefore, is not part of this TOP. Modifications, or replacement, of the originally
proposed filtration equipment is planned and any substantial changes to that design will be resubmitted to the Division for review. This project also added two 1.5 horsepower jet pumps
that will draw water from the new 5,000-Gallon Tank and pressurize the existing hydropneumatic tank (ST001). Prior operation of this system had the well pump pressurizing the hydropneumatic
tank. The new 5,000-Gallon Tank is identified as ST002 in the Division’s database. The new booster pumps have been added to the existing hydropneumatic tank facility (ST001) in the Division’s
database.
A permanent operating permit will be required, prior to April 30, 2026, for the new 5,000-Gallon Tank, Hydropneumatic tank pumps and the central arsenic filtration system. In order to
obtain an Operating Permit for the Arsenic Water Treatment Plant (TP001), the following sampling results will be required:
Four (4) separate sets of raw and treated water samples, taken a minimum of 24 hours apart including arsenic, iron and pH. Treated samples shall be alternated between the two filters
so that we have two finished water samples from each vessel.
Please note that the proposed Arsenic Water Treatment Plant (TP001) will have recurring sampling requirements for arsenic in the future. Additionally, other inorganics, metals, water
quality parameters, or increased lead and copper sampling may be required in the future. These compliance samples will be required post-treatment and will be decided at the issuance

We have received the following information for the 5,000-Gallon Tank:
Original design drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
VOC samples showing satisfactory results after tank interior coating.
We have determined that all conditions for operating permit issuance have been met. On this basis, a Temporary Operating Permit for the 5,000-Gallon Tank (ST002)is hereby issued as constituted
by this letter. You may now place the 5,000-Gallon Tank in service in your water system.
This Temporary Operating Permit expires on April 30, 2026. Prior to April 30, 2026, you must submit a permanent operating permit request that includes contract record drawings of the
central arsenic treatment equipment, satisfactory bacteriologic samples and the arsenic water sample results as explained previously in this letter. A dormant deficiency, G001, will
be added to your water system related to “unapproved facility in service” to track the expiration date of this temporary operating permit. An updated IPS report is available at https:\\waterlink.utah.gov.
