Dear Chris Thompson:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Malcolm Springs 5.0 MG Tank from your consultant, Steve Hansen of
SunriseEngineering on March 7, 2025. The Division issued Plan Approval for this project on August 16, 2022.
Our understanding of the project was the construction of a new 5.0 MG concrete storage tank
identified as (ST007) in the Division’s database. This new tank is replacing the Malcolm Spring
Upper Reservoir (ST001) and the existing Malcom Spring Lower Reservoir (ST002). The Upper
Reservoir tank (ST001) has been abandoned and demolished. The lower reservoir will now be
abandoned and demolished with the completion of this new 5.0 MG storage tank.
We have received the following information for Malcolm Springs 5.0 MG Tank (ST007):
Certification of Rule Conformance with plan approval conditions by a professional engineer
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Malcolm Springs 5.0 MG Tank is hereby issued as constituted
by this letter. You may now place the Malcolm Springs 5.0 MG Tank in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
