Box 221 Orderville, Utah 84758 Re:Notice of Approval to Proceed with the Public Notification Process Sparky’s Gas N’ Go, located at 94 West State Street, Orderville, Utah Facility Identification No. 6000142, Release Site NNZ Dear Mr. Esplin: The Division of Environmental Response and Remediation (DERR) has completed its review of the Corrective Action Plan (CAP) for the above-referenced facility, which Rockwell Solutions, Inc., submitted to the DERR on January 24, 2025. You may now proceed with the public notification process as proposed or as modified by the following instructions: 1. Provide notice to the potentially affected public in both the construction area and the treatment area if they are at different geographical locations; 2. Provide notice to the local health department and submit documentation of their approval for your proposed cleanup activities; and 3. Obtain any necessary permits or approvals from other regulatory agencies and submit the resulting documentation. Please provide this office with proof of public notification and other documentation outlined above within 14 days of receiving this letter. Once the additional documentation has been received, the DERR will proceed with the final approval process so that implementation of the proposed corrective action plan may begin. This letter does not give approval to begin cleanup activities. This approval is solely for starting the public notification/comment process. In the event that significant Facility ID# 6000142 Page 2 concerns are raised during this notification process, we may need to meet with you, your consultant, and the interested public to discuss any concerns regarding the cleanup at the site.
