Please see the site map attached (Appendix I). Incident Background: On 18 Oct 24, at 1324 ET, Penske (Roadside) contacted ERTS to report a vehicle release located in Salt Lake City, UT . It was reported that approximately twenty-five gallons of diesel fuel were released from unit/truck #450438 due to road debris . Vehicles affected were still leaking fuel and held a maximum of two hundred gallons of fuel. The release impacted asphalt and soil. No storm drains or waterways were reported to be impacted by the release . It was later determined that up to 150 gallons were released to the asphalt roadway and adjacent soil. Emergency Response: On 18 Oct 24, Penske retained Emergency Response and Training Solutions (ERTS) to manage and oversee environmental operations . ERTS dispatched Clean Harbors out of Salt Lake City, UT to conduct cleanup operations. Regulatory Notifications: ERTS 11231 Phillips Industrial Blvd. Bldg. 1, Ste 300 • Jacksonville, FL 32256 • USA phone number: 440.349.2700 I fax number: 440.349.6713 ertsonline.com E l:R IS Emergency Response and Training Solutions Pursuant to the State spill reporting statute Utah Administrative Code , R3 l 5, Rules 315-9-1, 315- 9-4 , on 18 Oct 24 , at 1355 ET, ERTS contacted the Utah Department of Environmental Quality and operator Mike Zucker issued number 19353 for this incident. Corrective Action: On 18 Oct 24, at 1520 ET, ERTS contacted the driver for some details , and he said it was a 150- gallon spill mostly impacting soil. At 1605 ET , Clean Harbors arrived on site and assessed the impact areas . Clean Harbors confirmed that 150 gallons of diesel were released impacting a 100ft by 1 Oft area of soil and asphalt. The asphalt impact was treated by applying granular absorbent, working into the surface, and sweeping the waste into the excavation area . The soil impact was covered in plastic and secured for later excavation . No waste was removed from the site on this day . On 27 Jan 25 , Clean Harbors returned to the site to complete the remediation . The crew arrived at 1030 ET and began setting up the equipment. The area impacted measured 100ft by 1 Oft varying from 6 to 10 inches deep . Excavation was completed on 27 Jan 25, and the replacement of the removed material with backfill was completed on 28 Jan 25 . On 29 Jan 25, the contractor advised they completed the excavation of the site and one and half roll offs were generated and contained the impacted soil. All on-site work was completed on 29 Jan 25 . Please see the attached provided site photos (Appendix 2). Waste Disposal: On 30 Jan 25 , Clean Harbors transported the roll offs of impacted solids to Clean Harbors in Grassy Mountain for proper disposal. Please see the attached disposal documents (Appendix 3). Conclusion and Recommendations: The release has been completely remediated, and no evidence was found that suggests any significant environmental impact remains on the site. ERTS suggests that this project be closed, and no further remedial action be taken . ER TS and Penske appreciate your assistance in this matter .
