Hunt Corporate Drive Lowell , AR 72745 Incident Description: Diesel Fuel Release Incident Time/Date: 1445 ET / November 06 , 2024 Incident Location: Interstate 80 East (Exit 111) Incident City, State: Salt Lake City, UT Incident County: Salt Lake County Utah DEQ Reference# 19371 ERTS Project# 221653 To Whom It May Concern, The following serves as the final report detailing environmental cleanup and remediation activities for the incident that occurred at the above referenced location on November 06 , 2024 . Please see the site map attached in Appendix I. Incident Background: On November 06 , 2024, at 1445 ET, approximately 80 gallons of diesel fuel released from the saddle tank of J.B. Hunt Transport Services truck #207084 due to a road debris strike . The released fuel impacted the soil beyond the concrete shoulder. No drains or waterways were reported impacted by this release. Emergency Response: On November 06 , 2024 , at 1644 ET, J.B . Hunt Transport Services retained Emergency Response and Training Solutions (ERTS) to manage and oversee environmental operations . ERTS di s patched Clean Harbors of Salt Lake City, UT to conduct cleanup operations . Regulatory Notifications: On November 06 , 2024 , at 1704 ET, ERTS notified the Utah Department of En vironmental Quality of the relea se . Edwardo received the call and issued reference number 193 71. ERTS 11231 Phillips Industrial Blvd. Bldg. 1, Ste 300 • Jacksonville, FL 32256 • USA phone number: 440.349.2700 I fax number: 440.349.6713 ertson line .com = ERTS Emergency Response and Training Solutions Corrective Action: On November 06 , 2024 , Clean Harbors arrived onsite and observe an area measuring approximatel y 50ft-by-l Oft of soil impacted by the released fuel. The impacted soil was secured for remediation at a later date . On January 06 , 2025 , EnviroServe arrived onsite to complete remediation of the impacted soil. EnviroServe personnel excavated an area measuring approximately 50ft-by-1 Oft to the maximum depth of 6 inches. EnviroServe personnel recovered approximately 27 yards of diesel impacted soil and was containerized into two roll-off boxes . Four confirmation samples were collected and later ORO analyzed . The Analytical Report outlining sample results is provided in Appendix 2. The excavated area was back filled with clean soil restoring the site to pre-incident condition. The generated waste was transported offsite for disposal. Photos provided by Clean Harbors are attached in Appendix 3 . Waste Disposal: On January 31 , 2025, two roll-off boxes containing approximately 22 . 78 tons of diesel impacted soil were transported to Republic Services located in Grantsville , UT for disposal. Please see the Waste Disposal Documentation provided by Clean Harbors attached in Appendix 4. Conclusion and Recommendations: The release has been completely remediated and no evidence was found that suggests any significant environmental impact remains on the site. ERTS suggests that this project be closed, and no further remedial action be taken. ERTS and J.B .
