HomeMy WebLinkAboutDDW-2025-003208 April 3, 2025 Joni Delayne Flint Pine Valley Mt Farms 100 East Wells Fargo Road Brookside, Utah 84782 Subject:Operating Permit, Pine Valley Mt Farms Tank Refurbishment (ST001) Project; Pine Valley Mt Farms, System #27061, File #13531, SRF #3F1890 Dear Joni Flint:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Pine Valley Mt Farms (Tank Refurbishment Portion) Project from Glen Carnahan, Alpha Engineering, on April 2, 2025. Previously, the Division originally received the plans and specifications for the Pine Valley Mt Farms Project from Jake Heward, Alpha Engineering, on September 19, 2023. Written review comments were provided to you and your consultant on October 11, 2023. The project was modified and included only the water line replacement and the tank floor replacement and refurbishment. The Division received the revisions and a response to the review comments from Colter Lund, Alpha Engineering on July 26, 2024. The Division issued Plan Approval for this project on August 15, 2024. DEQ District Engineer, J. Paul Wright, P.E., conducted an onsite construction inspection of this project on March 3, 2025. Tank Refurbishment Since this is the only tank on the system, it needs to be back in service as soon as possible. This Operating Permit is only for the tank refurbishment portion of the project. Below is a description of the project. The existing 425-000 gallon water storage tank was taken offline. The initial plan was to replace the floor, but upon inspection, it was determined to still be in good condition. A new manway was constructed, a new air vent installed on top of the tank and a new sump for drainage. Once this was complete, the interior of the tank was sandblasted and recoated with approved ANSI/NSF 61 epoxy coating and was allowed to cure properly. We have received the following information for the Pine Valley Mt Farms Tank Refurbishment (ST001) Project: Certification of Rule Conformance with plan approval conditions by a professional engineer: The tank floor was not replaced but inspected and determined to be in good condition. The manway design was altered. The size of the sump opening was changed. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. VOC samples were collected and all analytes were determined to be Non-Detect We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Pine Valley Mt Farms Tank (ST001) is hereby issued as constituted by this letter. You may now placeTank(ST001) back in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. If you have any questions regarding this Operating Permit, please contact J.
