Subject: RTCR Level 2 Assessment Brigham City UTAH02004
Mr. Waite, thank you for meeting with Brian Pattee and me on November 19th, 2025, for the Level
2 assessment of the Brigham City Water System.
Observations:
This Level 2 Assessment was triggered by multiple TC+/EC- samples in July and August 2024.

Brigham City is a mid-sized city located in Box Elder County, Utah with a population of over
20,000 people and 5880 connections.

During the course of the investigation, it became apparent that the most likely cause of the
unsatisfactory samples was inexperienced personnel using improper sampling technique.
Brigham City was able to provide chlorine residual data which showed adequate secondary
disinfection at all times and locations samples were taken.

Conclusions:
1. The most likely cause of the unsatisfactory samples appears to be improper sampling
technique being employed by new or inadequately trained staff.

Recommendations:
1. Recommend continued training of all employees on proper sampling protocols including proper
site selection, disinfection, flushing, bottle handling and delivery to the lab.
