If these deficiencies have been corrected, I kindly request that you provide detailed information regarding the actions taken to address them, accompanied byevidence available for verification purposes.
If these issues remain unresolved and additional time is required for their correction, I invite you to click on the provided link to request a Corrective Action Plafford you more time to address these matters effectively.
Please be advised that violations for the outstanding significant deficiencies are scheduled to be assessed on February 20, 2025. To prevent any adverse impincluding an additional 50 points for each violation, I urge you to contact me before the specified date.
