Department of Environmental Quality 620 South 400 East #400 St. George, UT 84770 Re: Operating Permit – Pine Valley Mt Farms Tank Floor Replacement and Refurbishment Project (ST001), Pine Valley Mt Farms System #27061, File #13531, SRF #3F1890 Dear Paul: In accordance with R309-500-9 “Issuance of Operating Permit,” we are providing the following information and are requesting an Operating Permit for the Pine Valley Mt Farms Tank Floor Replacement and Refurbishment Project, Brookside, Utah. • We certify that the tank has been refurbished in accordance with the plans and specifications provided by this office and approved by Utah Division of Drinking Water in a letter dated August 15, 2024. • A Drawing of Record has been prepared, and is included with this correspondence. Copies of the Drawing of Record have also been submitted to Pine Valley Mountain Farms Water Company. Any changes were in accordance with applicable Utah Administrative Code, R309-500 through R309-550. There were changes made during the construction of the project that are summarized below: o The new manway design was modified through the submittal process. The new manway has been updated in the drawing of record. o The sump opening dimensions were modified from originally panned 30” x 60” to 28” x 20”. o The existing tank floor was initially planned for replacement; however, after evaluation, it was deemed to be in good condition and was retained. o An additional interior beam was removed and replaced. • The system was flushed, disinfected appropriately and sampled. The satisfactory results from the bacteriologic samples and VOC samples are enclosed. Mr. J. Paul Wright P.E.
