The above water system requests approval of standard installation drawings and specifications for water
transmission and distribution lines per R309-500-7. For each water line:

Directions: Complete the above application and attach standard installation drawings and specifications meeting the requirements of
R309-550 for construction of water lines. The application must be signed by the designated water system contact or by the professional
engineer designated to oversee water system design. The drawings and specifications must be signed, sealed, and dated by a professional
engineer licensed by the state of Utah. Submit the completed application and standard installation drawings and specifications to the
Division of Drinking Water.

System specific standard installation drawings and specifications are attached for approval, or
System has formally adopted the current applicable and unamended APWA Part 5 standard installation
drawings and Division 33 specifications.
1.a hydraulic analysis will be competed by the designated P.E. to assure compliance with minimum sizing, pressure, andflow requirements;
2.flushing, disinfection, and coliform sampling will be completed according to ANSI/AWWA standards before use of thewater line; and
3.as-built or record drawings will be maintained.
