Director March 18, 2025 Wesley Vanvalkenburg Midvale City Water System 8196 S 700 W Midvale, UT 84047 Subject: Notice of Source Grouping, Change in Sampling Location for Midvale City Water System; UTAH18017 Wesley Vanvalkenburg: This letter serves as notification that due to the configuration of the Hancock Replacement Well (identified as WS008) and the Million Gallon Well (WS001), these two sources qualify for collective chemical sampling at the outflow of the holding tanks (location identified as TP009 “PCE Blend Site”). The chemical monitoring requirements mandated under R309-205 should now be collected from the PCE Blend Site (TP009), rather than from the individual sources themselves. Chemical samples collected from the PCE Blend Site (TP009) shall be collected at times when both sources are represented. The Hancock Replacement Well (WS008) contains elevated levels of tetrachloroethylene (PCE). A blending plan to reduce PCE concentrations entering the distribution system was approved by the Division of Drinking Water (the Division) on April 6, 2023. The plan requires quarterly volatile organic compound (VOC) monitoring at both the Hancock Replacement Well (WS008) and the Million Gallon Well (WS001) so that if PCE concentrations increase, appropriate operational adjustments can be made. This requirement remains in place. The updated monitoring requirements for Midvale City Water System are given below. Please ensure that the chosen laboratory processes PFAS samples using one of these methods. Additionally, the Division has resources to assist water systems in completing the PFAS initial monitoring requirements.
