HomeMy WebLinkAboutDDW-2025-002902 March 28, 2025 Chad McPherson Leamington Town Box 38106 Leamington, Utah 84638 Subject:Operating Permit, 2024 Culinary Water Improvements Project, 2024 Concrete Tank (ST004); Leamington Town, System #14012, File #13666 Dear Chad McPherson:The Division of Drinking Water (the Division) received a request for an Operating Permit for the 2024 Concrete Tank project from your consultant, James Saunders, with Jones & DeMille Engineering on March 18, 2025. The Division issued Plan Approval for this project on April 22, 2024. DEQ Central District Engineer, John Chartier, conducted an onsite final inspection of this project on March 25, 2025.Our understanding of the project is that a new 200,000-gallon concrete tank has been constructedadjacent to and at the same elevation as the three (3) existing storage tanks. The project also included approximately 8,500 feet of new 6-inch diameter well supply pipeline and approximately 4,100 feet of new 12-inch diameter transmission piping. The new storage tank is identified as the 2024 Concrete Tank (ST004) in the Division’s database We have received the following information for the 2024 Concrete Tank (ST004): Certification of Rule Conformance with plan approval conditions by a professional engineer. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the 2024 Concrete Tank (ST004)is hereby issued as constituted by this letter. You may now place the 2024 Concrete Tank in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
