Broadway STE 1250 Salt Lake City, Utah 84111 DESCRIPTION OF PERMIT MODIFICATION This minor modification of Ground Water Discharge Permit No. UGW210005 is in response to the signed letter dated January 16, 2025 (Document No. DWQ-2025-002056), which informed the Utah Division of Water Quality (“Division”) that MANGO II-Milford, LLC had acquired the Murphy-Brown, LLC assets located in Beaver, Iron, and Millard counties (sometimes referred to as “Smithfield” and/or “Smithfield Hog Production”). The written notice provided to the Division fulfills the requirements of Utah Admin. Code R317-6-6(6.18)–Ground Water Permit Transfer. This name/ownership change has been reflected in Ground Water Discharge Permit No. UGW210005 and this Statement of Basis. This is considered a minor modification. The only significant, proposed change to Ground Water Discharge Permit No. UGW210005 was a modified Closure Plan, which is being assessed as a separate modification for all permits now held by MANGO II-Milford, LLC. In addition, while Ground Water Discharge Permit No. UGW210005 currently has an expiration date of June 2, 2024, per Utah Admin. Code R317-6-6(6.7)-Ground Water Discharge Permit Renewal, if a permit expires before an application for renewal has been acted upon by the Director, the permit will continue in effect until it is renewed, extended or denied. The Division has received the renewal application and staff are in the process of reviewing the information provided. PERMIT DURATION It is recommended that this permit modification be effective through the current permit expiration date, June 2, 2024 (see above).
