Dear Mr. Tao:
In accordance with W ork Assignment 06 Atlas Technical Consultants (Atlas) abandoned two
groundwater monitoring wells and one sub-slab vapor monitoring point (SSVMP) at the above
referenced site. The abandonment efforts took place on March 7, 2024. A map showing the site
layout and the decommissioned well locations is presented as Figure 1. Given that the wells were
less than 30 feet deep, a state of Utah licensed well driller was not required to provide oversight.
Atlas personnel performed all well abandonment activities. The abandonment activities of
groundwater monitoring wells MW -21, MW -22, and SSVMP-1 are described in the following
paragraphs.
Decommissioning involved the proper abandonment of two 2-inch diameter groundwater monitoring
wells and one SSVMP. Abandoning the monitoring wells included removing the well casings to a
minimum of one foot below grade. The well casings were then backfilled to approximately one foot
below grade using bentonite chips. The bentonite chips were then hydrated. The wells were then
finished flush with the existing surface with concrete. All areas were then swept and cleaned. Atlas
personnel properly disposed of all recovered well materials. The vapor pin decommissioning of the
SSVMP included removing the vapor pin, then finishing the surface to match the surrounding
concrete. Photographs taken during the abandonment activities are presented in Appendix A.
