Sawyer Orderville Town Water System PO Box 165 Orderville, Utah 84758 Subject:Plan Approval, Orderville Town Water Line Replacement and Looping Project; Orderville Town Water System, System #13008, File #19610, SRF #3F2023 Dear Verl Sawyer:The Division of Drinking Water (the Division) received the plans and specifications for the Orderville Town Water Line Replacement and Looping Project from Paul Anderson, Sunrise Engineering, on February 19, 2025. Written review comments were provided to you and your consultant on March 13, 2025. The Division received the revised plans and specifications on March 18, 2025. This project has met the conditions for receiving plan approval. Our understanding of the project includes the installation of 1,585 linear feet of 8-inch diameter PVC C900 DR18 pipe. Part of the project will be the replacement of some existing 4-inch diameter water line. The remainder will loop this water line in two different locations back to the water main located on US Highway 89. The project includes reconnecting existing water service lines, the installation of 2 fire hydrants, valves, and appurtenant fittings. We have completed our review of the plans and specifications, stamped and signed byBlaine C. Worrell, P.E., dated February 18, 2025, and found they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Orderville Town Water Line Replacement and Looping Project are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before these water lines may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. Federal SRF Program The project, as described herein, is generally eligible for assistance under the Drinking Water Construction Assistance Program. All change orders will need to be reviewed and approved for compliance with the Drinking Water Rules and the conditions of this construction approval. The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding recipient, by ensuring that the contract meets requirements for funded projects. Federal regulations require that a concerted effort must be made to encourage participation of small and disadvantaged business enterprises (DBE) by providing requisite information, including plans and specifications on the project. In order to provide opportunities to DBE subcontractors, utilization goals for Minority-owned Business Enterprises (MBE) and Women-owned Business Enterprises (WBE) have been determined to be 4% and 2%, respectively. As outlined in the contract documents, the prime contractor must submit a list of prepared MBE/WBE subcontractors to be used on this project and documentation of solicitation efforts with the bid. The prime contractor's efforts to provide opportunity to DBE's must be reviewed and approved by this office before the contract can be awarded.
