Director of Drinking Water November 19, 2024 Melissa Hughes Pinion Forest SSD PO Box 38 Duchesne, UT 84021 Dear Melissa Hughes: Subject: Disinfection Corrective Action Plan Closure UTAH07073-2024-01 On June 14, 2022, Pinion Forest SSD and the Division of Drinking Water entered into a Disinfection Corrective Action Plan (DCAP). As of November 07, 2024, Pinion Forest SSD has successfully received a Temporary Operating Permit (TOP). The DCAP is now officially closed, as the system now has a TOP, and the water system’s rating will remain at an approved status with 0 Improvement Priority System (IPS) points. The Division of Drinking Water recognizes that addressing deficiencies and violations can be difficult and often requires significant financial and personnel resources. We would like to thank Pinion Forest SSD for the efforts put forth in resolving the items, for your continued commitment to maintaining reliable infrastructure, and for ensuring the safety of the drinking water provided to your customers. Please find enclosed current copies of the closed DCAP, monitoring schedule, and IPS report to help Pinion Forest SSD maintain compliance and an approved status.
