I.I PURPOSE AND APPLICABILITY
lntermountain Regional Landfill (lRL) operates under Title V Operating Permit No. 4900334002,
issued to IRL on December 20,2023. Beginning in February 2023, IRL began operating the
LandfillGas Collection System (LGCS) which includes 39 wellheads, two generators (one primary
and a backup), two blowers (one primary and a backup), and a candlestick flare. An additional
nine wellheads (four East Bench and five C-Lateral) were brought online in February 2024 and
three more wellheads (C-Lateral) in September 2024. Routine monitoring tasks required by 40
CFR S 62 Subpart OOO, 40 CFR $ 63 Subpatt AAAA were completed as required.
This report encompasses January 1,2024 through December 31, 2024. This report contains the
items specified in 40 CFR S 63.1981(h) and is solely intended for use by the Utah Department of
Air Quality (DAO) for purposes of compliance assessment and by IRL as a facility compliance
document. lt should be recognized that this report presents compliance information specific to the
timeframe herein. The compliance status of the facility is certified in Appendix A.
1.2 BACKGROUND
Based on Tier 2 Landfill Gas Testing dated July 2019 (prepared by Kleinfelder) and submitted to
the Utah DAQ in August 2019, the non-methane organic compound (NMOC) emissions exceeded
the 50 Mg/yr NMOC threshold. A LGCS design was submitted to the Utah Department of
Environmental Quality (DEO) in August 2020 and LGCS construction began in August 2021. The
Approval Order (AO) for the candlestick flare and associated generators was provided to IRL by
Utah DAQ in November 2022 followed by the Title V Operating Permit issued on December 20rh,
2023.
Over the course of the reporting period in the first quarter of 2024, the facility has incrementally
increased the height of the top deck, multiple times in select locations. Each lift on the top deck
required wellheads to be raised prior to Municipal Solid Waste (MSW placement, subsequently
leaving the wellheads inaccessible. Once MSW was placed around the lifted wellheads, access
could be regained. Multiple wellhead locations suffered from missing data due to access safety
issues described above. As of March 2024, the top deck is up to grade with no additional lifts
planned.
1.3 STARTUP SHUTDOWN MALFUNCTION EVENTS
40 cFR SS 63.1981(hxl) - 63.re81(hX4)
IRL began recording collection system and control device startup, shutdown, and malfunction
events as required by 40 CFR $ 63 Subpart AAAA beginning in March of 2023, one month after
initial startup.
Shutdown of the flare has occurred periodically due to maintenance activities on the flare,
generators and/or collection field, as well as malfunctions (Table 1). As explained in section 1 .2
Background, nrlmerous system factors lead to unexpected automatic flare shutdowns throughout
the reporting period. However, no uncontrolled releases of hazardous air pollutants have occurred
during this reporting period with landfill gas remaining inside the landfill and collection system
when the flare was not operating. This is accomplished through closure of the block valve at the
flare after the header during every flare shutdown as confirmed by telemetry. Most unexpected

shutdowns occurred outside of standard operating hours, including weekends, and the system
was restarted the following work morning. IRL achieved a flare operational time of 96.3% in 2024.
1.4 LGCS WELLFIELD MONITORING
40 cFR SS 63.1e81(hxlXi) - 63.1e81(hxlXiii)
LGCS wellfield monitoring was performed monthly, in accordance with AO and 40 CFR S 63
Subpart AAAA requirements. The LGCS wellheads are monitored for gas flow and quality at least
once per month to comply with permit conditions and are balanced as needed to optimize gas
extraction from the landfill. Wellheads are monitored for temperature and static pressure for
compliance requirements, and oxygen and nitrogen for operating and tuning parameters (the
nitrogen/orygen compliance requirement has been removed from the newly effective Emissions
Guidelines 40 CFR S 60 Subpart Cf, 40 CFR S 62 Subpart OOO since June 21 ,2021, and 40
CFR S 63 Subpart AAAA since November 12, 2020).
Wellhead repairs and earthwork to improve access to select wells were conducted in early spring
when access could be established. lt is likely waste placement activities have collapsed multiple
wells at depths between 24 to 38-feet below ground surface (bgs). The following wells collapsed
at various depths as seen through pipe videos:
. G1-WB-04, G1-A1-02, G1-A1-04, G1-A1-07, G1-81-04, G1-81-05, G1-81-07, G1-C1-02,
G1-C1-05
Note that some of the collapsed wells occurred in late spring, resulting in initial readings for
wells (such as G1-C1-05) but no further readings. All broken wells were redrilled in August and
reconnected to the collection system in September. Wellhead data was collected in January
2024lrom 39 wells. ln February 2024, an additional 9 wellheads were brought online (east
bench and C Lateral) resulting in data from 48 wells in February through August 2024 (Figure
1). ln September 2024, an additional 3 wellheads were brought online (C Lateral) resulting in
data from 51 wells in September through December 2024. A number of wellheads are missing
data due to access and safety issues or out of compliance in this annual period as documented
in the following:
o January: G1-WB-20 was out of compliance with a positive pressure of 0.03 inches H2O.
Additionally, data was not collected from G1-WB-16, G1-A1-03, G1-A1-05, G1-41-06, G1-
A1-07, and G1-B1-02.
. February: Temperature at G1-WB-20 exceeded 145 degrees Fahrenheit with a result of
175.2 degrees Fahrenheit. Additionally, positive pressure was observed at G1-EB-05 and
G1-E8-06 with 0.07 and 0.3 inches H2O, respectively. Additionally, data was not collected
from G1-WB-16, G1-A1-03, G1-A1-05, G1-A1-06, G1-A1-07, G1-B1-02, and G1-81-06.
. March: Temperature at G1-WB-20 exceeded 145 degrees Fahrenheit with a result of 178.4
degrees Fahrenheit. Additionally, no data was collected from G1-A1-03.
. April: No data was collected from G1-A1-03 due to access issues. Note that collapsed
wells are not shown in the table.

August: On August 28,2024, it was identified the A Lateral became blocked between G1-
41-01 and G1-A1-02 and loss of vacuum occurred in all A Lateral wells except G1-A1-01.
Work began within five days to restore vacuum, but field adjustments had no effect on
restoring vacuum. As vacuum could not be restored within 15 days, HAL began a root
cause analysis which included three attempts to video locate the blockage and an attempt
to excavate the lateral. Note that collapsed wells and the blocked A Lateralwells are not
shown in the table.
September: The collapsed wells stated above were redrilled in August and reconnected in
September. The A Lateral wells which had been blocked have been temporarily
reconnected to vacuum via a three-inch bypass hose. The root cause analysis was
finished. G1-A1-02 was connected to vacuum but lacked a wellhead for measurements and
is not shown in the table. There were no exceedances by the end of the month on any well.
October: There were no wells with exceedances for temperature or pressure. G1-A1-02
was connected to vacuum but lacked a wellhead for measurements and is not shown in the
table.
November: There were no wells with exceedances for temperature or pressure. G1-A1-02
was connected to vacuum but lacked a wellhead for measurements and is not shown in the
table.
. December: There were no wells with exceedances for temperature or pressure. G1-A1-02
was connected to vacuum but lacked a wellhead for measurements and is not shown in the
table.
1.5 LANDFILL SURFACE EMISSIONS MONITORING
40 cFR S 63.1e81(hxs)
Four quarterly (February, May, and September, and November 2024) Landfill surface emission
monitoring events were performed during this reporting period. Surface emissions were measured
across the Landfill surface using a Surface Emission Monitor (SEM) 5000 meeting the
specifications of EPA Method 21. The measurements were made in accordance with federal and
AO requirements.

1.5.1 February 2024 Monitoring Event
The first quarter monitoring event was conducted on February 16,2024. The walking pattern was
conducted following a north to south traverse, as illustrated in Figure 2. A surface inspection was
performed concurrent with the surface emission monitoring to identify cracks, erosion fissures, or
other anomalies in the Landfill cap that may create escape pathways for methane. Field forms for
this quarterly monitoring event are provided in Appendix C. No exceedances were observed
during this monitoring event.
ln addition to the Surface Emission Monitoring event, all penetrations within the lined area of the
active landfill collection system footprint were monitored for methane. Two exceedances, 800
parts per million (ppm) and 957 ppm methane were identified at KO-02 and G1-C1-03,
respectively. IRL operators were immediately informed of the exceedances at the conclusion of
the Surface Emission Monitoring event. Corrective action wastaken on February 21,2024by
placing additional cover material around the penetrations and general area. The 1O-day followup
inspection was conducted on February 26,2024, with passing results below 500 ppm methane.
Additionally, the 30-day follow-up inspection was conducted on March 15, 2024, with passing
results below 500 ppm methane. Field forms for this quarterly monitoring event are provided in
Appendix C.
1.5.2 May 2024 Monitoring Event
The second quarter monitoring event was conducted on May 23, 2024.The walking pattern was
conducted following an east to west traverse, as illustrated in Figure 3. A surface inspection was
performed concurrent with the surface emission monitoring to identify cracks, erosion fissures, or
other anomalies in the Landfill cap that may create escape pathways for methane. Field forms for
this quarterly monitoring event are provided in Appendix C. No exceedances were observed
during this monitoring event.

1.5.3 September 2024 Monitoring Event
The third quarter monitoring event was conducted on September 5,2024. The walking pattern
was conducted following a north to south traverse, as illustrated in Figure 4. A surface inspection
was performed concurrent with the surface emission monitoring to identify cracks, erosion
fissures, or other anomalies in the Landfill cap that may create escape pathways for methane.
Field forms for this quarterly monitoring event are provided in Appendix C. No exceedances were
observed during this monitoring event.

1.5.4 November 2024 Monitoring Event
The fourth quarter monitoring event was conducted on November 20 and 21,2024. The walking
pattern was conducted following a east to west traverse, as illustrated in Figure 5. A surface
inspection was performed concurrent with the surface emission monitoring to identify cracks,
erosion fissures, or other anomalies in the Landfill cap that may create escape pathways for
methane. Field forms for this quarterly monitoring event are provided in Appendix C. No
exceedances were observed during this monitoring event.

1.6 VISUAL EMISSION MONITORING
Quarterly observations were conducted on March 8,2024, June 24,2024, September 5, 2024,
December 16, 2024. EPA Method 9 observations were conducted on stationary sources in
accordance with ll.B.1 .b.1 & 11.B.3.a.1 , of the permit. Observation data is provided in Appendix D.
Due to mechanical issues, no visual emission observation was conducted on Genset #1 in March
2024. Also due to mechanical issues, no visual emission observation was conducted on Genset
#1 or Genset #2 in December 2024, but a visual emission observation was performed on the
temporary trailer-mounted mobile generator(Appendix D). No exceedances were observed during
these monitoring events.
1.7 FUGITIVE DUST EMISSIONS
ln accordance with the conditions of Permit Provision ll.B.1.c, the facitity has implemented and
continues to implement the most recently approved fugitive dust control plan as required by R307-
309-6 and administers one or more of the contingencies, or Best Management Practices (BMP)
provided for the plan. IRL predominantly utilized water and designated traffic patterns at the facility
to reduce dust. Additionally, IRL applied magnesium-chloride to the dirt road surface leading to
the facility amounting to 80 tons on June 14,2024. Fugitive dust emission reporting forms are
provided in Appendix E.
I.8 SUMMARY
IRL operated in compliance with conditions mandated by the current Title V Operating Permit
during the period covered in this report, with the exception of an attached deviation description
concerning an incomplete/late Title V report and late notification of deviation. These conclusions
are based on the data collected by Hansen, Allen & Luce and/or provided by IRL that are included
in this report.
Ongoing generator issues associated with the flare, broken wellheads, and access/safety issues
have restricted data collection and compliance efforts. Brief operational exceedances of positive
pressure/temperature were able to be resolved within 60 days or less for G1-WB-20 (pressure) in
January, G1-WB-20 (temperature) in February/March, G1-EB-05 and G1-EB-06 in February, and
A Lateral wells past G1-A1-01 in AugusUSeptember. Access and safety issues which impeded
certain wellheads were fully corrected in March and April. All broken wells were redrilled in August
and brought online in September, although G1-A1-02 lacked a wellhead for wellhead

measurements. Besides G1-A1-02, no acc€ss issues, safety issues, or broken wells impeded any
measurements in Q4 2024.
Quarterly surface emission monitoring was performed in four quarters, and two exceedances
were detected in Q1 which were corrected within five days and passed the follow up inspections.
Quarterly visible emission observations detected no exceedances, although Genset #1 was
unable to be measured in Ql and Q4, and Genset #2 was unable to be measured in Q4, due to
mechanical issues.
There were no recorded uncontrolled releases of excess landfill gas to the atmosphere from the
LGCS during the monitoring period. IRL achieved a flare operational time of 96.3% in 2024.
Based on conversation with DAQ compliance personnel, we are adjusting the reporting period for
this facility moving forward. The 1Q24 reporting period for this facility was documented and
submitted as part of the 2023 Annual Title V Report for the facility. This present report meets the
reporting requirement to document the work performed during the 2Q24 through 4Q24 time
period. Based on these two submittals, IRL has completed the monitoring, recordkeeping, and
reporting requirements for January through December of 2024. We anticipate submifting the next
semiannual monitoring report for the period of 1Q25 and 2Q25 in July 2025, along with the Annual
Title V Report for all of 2025 before March 30,2026. With these anticipated submittals, we are
redefining the semi-annual reporting period to be January through June and July through
December moving forward and the annual reporting period to be January through December
moving forward.

On behalf of ROC-Intermountain Regional MSW Landfill (lRL), Hansen Allen and Luce (HAL) submitted an annual Title V
,ort on March 27 , 2024 which was found by the Regulator to be incomplete. HAL submitted a revised annual report on May
, 2024 which triggered the violation of l.L.'l . A deviation report has not been submitted, triggering a violation of 1.S.2.c.
4.Was the deviation due to an "upset condition" as defined by Section ll of the permit?

5.Describe the probable cause of deviation:
overlap of the annual report period (April I , 2023 lo March 31 , 20211 and the submission deadline (March 3'l , 2024) has
confusion about how to certi! the days between ihe submission (if submitted before March 31, 2024) and the
deadline. lt also does not allow time for conections. HAL /lRL did not realize that a deviation had occurred since
original report was turned in before the deadline, so no deviation report was submitted within 14 days.
6.Describe any corrective actions or preventative measures taken:
HAL will submit the next annual report for a 9 month period at the end of 2024, which will reset the annual report
certiftcation period to a calendar year basis. This will allow 3 months for corrections before the submission
deadline of March 31.

ln accordance with the conditions of Permit provision ll.B.l.a, the facility has implemented, and continues
to implement, the most recently approved fugitive dust control plan R307-309-6 and administers one or
more of the contingency measures, or Best Management Practices, provided for in the plan as follows
(check those that apply):

E Minimizing transfer and drop height of materials during loading and dumping processes
