SUBJECT: LUST NFA - Closure Review Memo
R+L Carriers, located at 4375 West 1385 South, Salt Lake City, Utah
Facility Identification No. 4002691, Release Site OLO
A petroleum release from an Aboveground Storage Tank (AST) was reported to the Division of
Environmental Response and Remediation (DERR) on September 6, 2024. The release was reported to
the DERR based on Environmental Incident Report 19285. The extent and degree of contamination has
been fully defined and a No Further Action (NFA) review is hereby requested.
While an AST was receiving a delivery, the camlock on the fuel truck hose came loose and caused a
release of approximately 100 gallons of diesel. The release covered an approximate area of 20 feet by
100 feet.
Enviroserve was dispatched to the site to complete an emergency abatement. Granular absorbants were
applied to the diesel spill and then scraped up using hand tools. The absorbants were placed in steel
drums for proper disposal at the Sump and Trap cleaning facility in Salt Lake City.
No storm drains, waterbodies, or soil were impacted by the release.
Please initial this memo when your closure package review is complete. If you have questions,
comments or concerns with the closure, please discuss with the project manager or request a closure
Liberty Coe
committee discussion.
Once this memo is complete, the NFA letter will be initiated through Adobe Sign for final signatures.
