RE: Request for Operating Permit for the Leamington 200 K Tank

Dear John:

Regarding the referenced project, please note the following:

1. The project was completed according to approved plans and conditions.
2. One set of construction record drawings will be given to the Owner with completion of
transmission line portion of project.
3. Flushing and disinfection of the tank has been completed in accordance with the
appropriate AWWA/ANSI standard.
4. Only minor changes were made during construction and would not affect compliance with
the Utah Administrative code.
5. Copies of bacteria test results for the new tank are attached.
