ln accordance with Operating Permit provision 1.S.2.c, and Utah Administrative Code (UAC) R307-
415-5a(3XcXii), the fottowing deviation report is submitted:
1. Dates that the Deviation occurred: start: 10/2412024 End: 10/29/2024
2. Permit condition for which deviation occurred:
Permit condition ll.B.10.c(2) states that the "Emergency stationary ICE may be operated for
the purposes specified in paragraph (a) for a maximum of 100 hours per catendar year."
3. Describe the Deviation that occurred:
The facitity had a scheduted etectricat outage for 9 days from October 11-20,2024. The
emergency generator supporting overhead magnetic crane operations did not run during
this time, inctuding weekl,y scheduted readiness testing, as alt production was shut down
and the outage was ptanned. This emergency generator has a btock heater and control
panetthat continued to run off the genset batteries during this time. By October 21 the
emergency generator's batteries were dead. The facitity jumped the batteries on October
24,2024,neartheendof shiftandgottheunitstarted. Thefacitityneededtorunthe
emergency generator in order to charge the batteries, but once the site had started the
engine they faited to communicate to the next shift when to turn off the unit. As a resutt the
emergency generator continued to run untit it was turned off 5 days later. Prior to this event,
the emergency generator had run for 34 hours for maintenance checks and readiness
testing f rom January 1 through october 20,2024. This event resutted in an additionat 120
hours of operation, for a totaI of 154 hours of non-emergency operation this catendar year.

5. Describe the probabte cause of the deviation:
The facitity faited to devetop a process for recharging the emergency generator's battery
system. Consecutive shifts did not pass down communication that the emergency
generator had been turned on. Teammates were not trained to observe the generator "on"
status and faited to see that the unit was running.

6. Describe any corrective actions or preventive measures taken:
A process for charging the emergency generator batteries witt be developed that inctudes
monitoring the runtime of the unit during any required charging and teammates witt be
trained on this process. Responsibte teammates (receiving department) witt receive
training to monitor the generator's operationaI panet inside the buitding so they are aware of
when the generator is in operation vs. standby mode.
