RE: NOTICE OF RECIPROCAL RECOGNITION OF LICENSE FOR USE IN UTAH
NRC Radioactive Material License Number 39-35044-01, Amendment 03

Dear Mr. Rogers:
In accordance with EnergySolutions, LLC’s request dated January 28, 2025 (fee payment received
January 23, 2025) and Utah Administrative Code R313-19-30, the Division of Waste Management and
Radiation Control (Division) hereby authorizes the licensee to use radioactive material at temporary job
sites within the State of Utah, provided additional work episode notification and that the job sites are not
in areas under exclusive Federal jurisdiction. The use of radioactive material must be conducted in
accordance with the representations in EnergySolutions, LLC’s letter, Utah Radiation Control Rules, and
EnergySolutions, LLC’s recognized NRC radioactive material license.
Following the licensee’s receipt of this authorization, the Division requires written notification at least
three days prior to engaging in any licensed activity within the State of Utah. Permission may be
granted by the Director of the Division to proceed sooner in the case of undue hardship.
The licensee should notify the Division in a timely manner of any change to the information provided in
the initial reciprocity request, and report deviations from reported work episode notifications to the
Division as soon as they are known. This authorization is not effective after December 31, 2025, or
180 possession days in a calendar year, whichever occurs earlier.
