HomeMy WebLinkAboutDWQ-2025-002355April 3, 2025 Jade Dalton, Owner Lost Creek Hogs LLC PO Box 189 Circleville, UT 84723 jadedeandalton@gmail.com Subject: Approval to Use - Secondary Waste Containment Evaporation Basin Lost Creek Hogs LLC Dear Mr. Dalton: On October 23, 2024, the Division of Water Quality (“Division”) received a new set of plans and specifications for the Secondary Waste Containment Evaporation Basin (“Pond 2”) from your consultant, Joel A. Myers, P.E., of GEM Engineering, Inc. The Division reissued the construction permit on November 7, 2024. GEM Engineering, Inc. completed the soils testing, and the Division received the test results and the various forms for the HDPE Lining in an email on February 3, 2025. However, the drawing which showed the HDPE panel placement and seaming was not included at that time. On March 11, 2025, I received an email from Keston Johnson of Mountainland Supply Company that included that drawing. The Division conducted a final inspection of Pond 2 on March 26, 2025 (report attached). The Division has also completed a review of the quality control documentation. Based on the final inspection and review of the quality control documentation, the Division approves the use of the Secondary Waste Containment Evaporation Basin.
