Dear Mr. Dawson:
On January 10, l99O authorized representatives of the Utah Bureau of Solid and
Hazardous Waste conducted a compliance inspection at your facility.
Please be advised of the following:
1. R450-4-2(a) of the Utah Administrative Code requires that items I and J of the
Uniform Hazardous Waste Manifest be completely filled out for all shipments of
hazardous waste.
2. R450-5-9(a) of the Utah Administrative Code requires small quantity generators
of hazardous waste to post emergency help information next to the facility
telephone.
By March 2, 1.990 you are hereby requested to provide this office with a written
account of your monthly rate of hazardous waste generation for the years 1986,
1987, and 1988.
Thank you for your cooperation during the inspection. Questions may be directed to Jim
